Job Summary
Join our compassionate healthcare team as a Hospice Admissions Nurse, where you will play a vital role in ensuring patients receive timely, respectful, and comprehensive end-of-life care. In this dynamic position, you will assess new patient referrals, coordinate admission processes, and collaborate with interdisciplinary teams to deliver personalized hospice services. Your expertise in critical care, hospital medicine, and patient assessment will help facilitate smooth transitions from hospital or other care settings to hospice care, providing comfort and dignity to patients and their families. Conduct thorough patient assessments upon referral, including vital signs, medical history review, and physiological evaluation to determine eligibility for hospice care.
Manage intake processes efficiently by coordinating with hospitals, clinics, and other healthcare providers to facilitate seamless admissions.
Develop individualized care plans based on comprehensive diagnostic evaluation and patient needs, ensuring adherence to health regulation policies and HIPAA privacy standards.
Collaborate with interdisciplinary teams—including physicians, social workers, and therapists—to coordinate discharge planning and post-acute care services.
Maintain accurate medical records using electronic health records (EHR) management systems; document all assessments, interventions, and communication thoroughly.
Educate patients and families about hospice services, managing expectations with clear communication about the scope of care and symptom management strategies.
Monitor patient status continuously through vital signs assessment and clinical observation to adjust care plans proactively.
Facilitate utilization management by evaluating the appropriateness of services provided, ensuring compliance with managed care protocols.
Participate in case management activities such as managing patients as a nurse case manager, overseeing ongoing care needs across inpatient and outpatient settings.
Ensure compliance with hospital medicine standards and health regulation policies while delivering patient service that emphasizes compassion and dignity.

Header
Experience

Proven experience in critical care environments such as ICU or Level I Trauma Centers is highly preferred.
Hospice RN experience.
Hospice admissions experience.
Extensive background in hospital medicine, emergency medicine, or post-acute care settings including inpatient or outpatient clinics.
Demonstrated expertise in medical-surgical nursing, infectious disease care, behavioral health, geriatrics, or pediatrics enhances your suitability for this role.
Strong knowledge of physiology, medical terminology, HIPAA regulations, and medical records management is essential.
Prior experience managing patients through intake processes as a nurse case manager or discharge planner is highly desirable.
Familiarity with electronic health records (EHR) systems and utilization management practices ensures efficient documentation and compliance.
Experience working within managed care frameworks or health regulation policies supports effective coordination of services across multiple settings.
Excellent patient service skills combined with the ability to perform detailed patient assessments—vital signs monitoring included—are critical for success in this role.
Inpatient experience in acute care environments such as trauma centers or emergency rooms provides valuable insight into complex patient needs.
