Full Job Description

MISSION:

POSITION SUMMARY:

Under the supervision of the Wraparound Program coordinator, The Facilitator is responsible for leading the Child and Family Team Meetings (CFTM) in providing Wraparound services to assigned youth and their families. Facilitator coordinates all facets of case needs including helping families use their voice, choice, and strengths to develop a support plan with the input from CSW/Probation officers and court mandates. A Facilitator seeks out and organizes client, family and CFT input to complete all case documentation including (but not limited to) Strength Summary, Matrix, Crisis and Safety Plan, Progress Notes, CFT Meeting notes and any other required documentation.

ESSENTIAL JOB FUNCTIONS:

Manage a caseload of 10 to 12 families.
Contact newly assigned referral family within 24 hours and secure intake appointment.
Establishes relationships and maintain effective communication with clients, CSW/probation officers, family members, and staff. Provide direct and timely services to children and families and maximize the involvement of all persons in the implementation of plans.
Completes initial Child and Family Matrix and safety plan to ensure that all strengths and underlying needs are identified within 30 days of intake. Facilitates ongoing child and family team meetings and continuously reassesses the needs of the child and family with the team and modifies the Matrix as needed (typically every 4 to 6 weeks)
Meet required productivity standards set forth in the Fiscal Year Production Days Schedule (indicates all expected monthly and annual productivity expectations)
Demonstrated adherence to Drew CDC and DMH contractually required note and form submission standards (includes all progress notes submitted in EHR within 48 hours
Available 24/7 (on rotation schedule) for crisis situations either over phone or in the field, submitting all required documentation related to crisis which includes SIR’s, reporting Suspected Child Abuse immediately.
Apply the Integrated Core Practice principles, which includes community partnership, teamwork, cultural competence, respect, accountability, and continuous quality improvement.
Consistently attends and collaborates with other Wraparound Team members by attending Child and Family Team Meetings, individual/group supervisions, Wraparound Team Bi-Monthly meetings.
Decrease any barriers of families accessing MH services, to include providing services in the home, field, school, and/or office.

MINIMUM QUALIFICATIONS:

Bachelors in human services, Social Work or Psychology with relevant experience in the social services field.
Two years' experience with high-risk children/youth in either child welfare, probation, or mental health systems.
Must be able to demonstrate a valid CA driver’s license, registration, and insurance.

DESIRED QUALIFICATIONS:

Experience working in DMH community mental health agency.
Experience providing Mental Health Program.
Wraparound experience working in intensive services.
Experience in utilizing electronic health record systems (EHR).

ENVIRONMENT:

The worker is subject to both environmental conditions (OFFICE): Activities occur inside and outside. MONDAY - FRIDAY (IN OFFICE)
