What You'll Do

The RN Case Manager plays a vital role in fulfilling our mission to deliver compassionate, family-centered care. We serve adult and geriatric patients in their homes, delivering personalized support that fosters independence, dignity, and improved well-being—always treating each patient with the same compassion and respect we would offer our own family. In your role as RN Case Manager, you’ll champion high-quality, compassionate care that helps patients thrive at home—where healing feels most personal.

In addition to adult and geriatric patients , we also serve children where they need it most—at home and in their schools. By mentoring and supporting pediatric field staff, the RN Case Manger ensures that every interaction reflects clinical excellence, regulatory compliance, and deep empathy for the unique needs of young patients and their families.

Serving as a key connector between clinical and operational teams, the RN Case Manager also builds trusted relationships with families, school personnel, hospitals, and community partners to create a seamless, collaborative care experience that empowers our patients to thrive.

Required Qualifications

A current RN license in Pennsylvania is required.
Minimum of two (2) years clinical nursing experience in general nursing, with home health experience preferred.
Strong communication, organizational, and team-building skills
High emotional intelligence and a genuine ability to connect with people from all walks of life
Exceptional attention to detail and ability to manage multiple priorities with efficiency
Proven ability to work collaboratively and consistently support team goals
Familiarity with EMR systems (Kantime or similar)
A positive, energetic approach is key to success in this role.
