Full Job Description

Rapidly expanding aging service provider seeks full time Case Managers/Care Coordinators for its Aging Services program in Champaign county.

Duties and Responsibilities:

Performing in-home comprehensive assessments,
Performing hospital and LTC facility based assessments, including week-end rotation
Developing, monitoring and revising person centered plans of care
Providing linkage and referral to other social service organizations
Follow up activities, etc.
Adherence to program timeframes and policies a must.

Qualifications and Skills:

Candidate should possess a bachelor's degree in social science or health field or be a registered nurse. Formal work experience with the 60+ population can be substituted for bachelor's degree. One year of experience for one year of college, up to four years.
Master's degree preferred
Positions work out of the Urbana office
Extensive travel and dependable transportation with vehicle insurance required
Valid Driver’s license
Computer literacy
Strong organizational skills and the ability to work independently
Bilingual candidates preferred
