Full Job Description

Job Summary

We are seeking a dedicated and detail-oriented MDS (Minimum Data Set) Coordinator to join our team at Springdale Post Acute. In this vital role, you will oversee the comprehensive assessment and documentation process for residents, ensuring compliance with federal and state regulations, including those set forth by the Centers for Medicare and Medicaid Services (CMS). Your expertise will support accurate clinical documentation, optimize patient care planning, and facilitate seamless communication across interdisciplinary teams. This position offers an exciting opportunity to impact resident outcomes positively while working within a dynamic healthcare environment that values professionalism, accuracy, and proactive collaboration.

Responsibilities

Coordinate and complete the MDS assessments in accordance with CMS guidelines, ensuring timely and accurate documentation of resident conditions.
Review medical records, clinical documentation, and health information to ensure compliance with clinical documentation standards and regulatory requirements.
Collaborate with nursing staff, physicians, case managers, and other healthcare professionals to gather pertinent information related to resident health status and care plans.
Conduct thorough documentation review to identify gaps or inconsistencies in medical records, supporting quality improvement initiatives.
Maintain proficiency in EMR (Electronic Medical Records) systems such as Cerner, Epic, or Athenahealth to facilitate efficient data entry and retrieval.
Stay current on Medicare regulations, long-term care policies, ICD coding updates (ICD-9/ICD-10), CPT coding, DRG assignments, and other health regulation policies to ensure compliance.
Assist with discharge planning, hospice care coordination, and outpatient or inpatient care transitions as needed.
Support utilization management efforts by reviewing medical necessity criteria for services rendered in acute care settings or long-term care facilities.

Requirements

Proven experience with MDS coordination within a long-term care or skilled nursing facility environment.
Active LPN license; RN license preferred
Strong knowledge of CMS regulations, Medicare guidelines, ICD coding (ICD-9/ICD-10), CPT coding, DRG classification, and health regulation policies.
Familiarity with EMR/EHR systems such as Cerner, Epic, eClinicalWorks, or Athenahealth; proficiency in Microsoft Office Suite is preferred.
Critical care experience or ICU background is highly desirable; inpatient experience in hospitals or trauma centers is a plus.
Knowledge of clinical documentation improvement strategies and utilization review processes.
Excellent understanding of medical terminology, anatomy, physiology, and nursing practices related to patient care.
Ability to interpret complex medical records accurately while adhering to HIPAA regulations and state healthcare laws.
Strong organizational skills with attention to detail; capable of managing multiple priorities effectively in a fast-paced environment.
Certification in health information management or coding (e.g., RHIT, CCS) is preferred but not required.
