Overview

Join our dedicated team as a Patient Safety Care Specialist, where your compassionate approach and attention to detail will make a meaningful difference in the lives of patients with diverse needs. In this vital role, you will provide attentive care, ensure patient safety, and support individuals with cognitive disabilities, developmental disabilities, and age-related conditions such as dementia and Alzheimer's. Your proactive engagement will foster a safe, respectful environment that promotes well-being and dignity for every patient. This paid position offers an enriching opportunity to apply your caregiving skills in a dynamic healthcare setting committed to excellence.

Responsibilities

Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using manual handling techniques and equipment like Hoyer lifts.
Monitor patient conditions through vigilant observation of vital signs, behavior, and overall health status; report changes promptly to healthcare professionals.
Support medication administration and medication management according to care plans while adhering to HIPAA regulations and medical terminology standards.
Implement behavior management strategies and conflict resolution techniques to promote a calm environment for residents with neurodevelopmental disorders or mental health conditions.
Prepare nutritious meals, assist with meal intake, and maintain cleanliness in patient care areas following infection control protocols.
Document patient care activities accurately using EMR (Electronic Medical Records) systems; ensure detailed patient condition reporting for case management purposes.
Collaborate effectively with nurses, social workers, therapists, and other healthcare team members to develop and follow individualized care plans tailored to each resident’s needs.

Requirements

Proven experience working in assisted living facilities, nursing homes, or home care environments supporting seniors or individuals with disabilities.
Knowledge of Alzheimer’s care, dementia care, memory care, and hospice care practices; familiarity with caring for patients with cognitive disabilities is essential.
Certifications such as First Aid, CPR (Cardiopulmonary Resuscitation), Basic Life Support (BLS), and crisis intervention training are required; ongoing training preferred.
Ability to perform heavy lifting safely and handle manual transfers using equipment like Hoyer lifts; comfort with physical activity involved in caregiving tasks.
Strong communication skills for caregiver interactions, patient observation reports, conflict management, and collaboration with healthcare professionals.
Experience working with individuals with autism or intellectual disabilities; understanding of medical terminology enhances quality of care.
Valid driver’s license preferred for driving duties related to patient transportation or errands; familiarity with EMR systems is advantageous.
