Case Coordinator (FT)

Department: NC Programs

Reports To: Supervising Substance Abuse Counselor

FLSA Status: Exempt

Shift: Mon- Friday (8am-5pm)

SUMMARY

The primary role of this position is to provide case management and community support services for substance abusing women and their children.

The Case Coordinator plays a vital role in providing direct case management and treatment support to women struggling with substance use and their children. This position is focused on connecting families to community resources, developing individualized treatment and life goals, and advocating for services that improve overall quality of life.

This is more than just a job—it’s a chance to make a lasting impact on a vulnerable population and walk alongside families on their path to recovery and self-sufficiency.

What You’ll Do

Manage and monitor a caseload of clients receiving services (SACOT, SAIOP, Outpatient).
Conduct needs assessments and assist consumers in developing Person-Centered and Economic Self-Sufficiency Plans.
Coordinate resources and link women/children to community supports.
Provide supportive counseling and collaborate with paraprofessionals to implement skills-building activities.
Facilitate crisis intervention, discharge planning, and case staffings.
Advocate for consumers in Family Drug Treatment Court and with other agencies.
Maintain accurate, timely documentation including reports, logs, and incident reports.
Participate actively in team meetings, clinical supervision, and interdisciplinary collaboration.
Serve on an on-call rotation as First Responder for crisis support.

What You’ll Bring

✔ Education & Experience

Bachelor’s degree in Human Services (minimum 2 years of experience), OR
Master’s degree in Human Services (minimum 1 year of experience).

✔ Skills & Knowledge

Strong observational, communication, and counseling skills.
Knowledge of community resources and referral processes.
Ability to assess client safety and respond to crises.
Excellent written, verbal, and interpersonal communication skills.
Professional presence with a client-centered, empowerment-oriented approach.

✔ Other Requirements

Ability to work collaboratively on a multidisciplinary team.
Strong reasoning and problem-solving abilities.
Physical ability to meet job demands (including lifting up to 50 lbs. occasionally).
Willingness to work in a variety of environments, including exposure to outdoor conditions and challenging situations.
