Full Job Description

Job Summary

We are seeking a dynamic and compassionate Nurse Practitioner to join our healthcare team. In this vital role, you will provide comprehensive patient care in a private Physician-owned clinic specializing in internal medicine and medicine subspecialty. Your expertise will support diagnosis, treatment, and management of a wide range of medical conditions, ensuring high-quality, patient-centered care. This position offers an exciting opportunity to work with a variety of patient populationsutilizing advanced clinical skills and a collaborative approach to healthcare delivery.

Responsibilities

Conduct thorough physical examinations and assess patient health status through detailed history-taking and vital sign evaluation.
Develop and implement individualized treatment plans, including medication administration, injections, suturing, and wound care.
Manage acute and chronic conditions related to the practices specialty
Perform diagnostic evaluations utilizing EMR (Electronic Medical Records) systems like Epic or eClinicalWorks; order and interpret laboratory tests, imaging studies, sonography, and other diagnostic procedures.
Provide patient education on health coaching topics including nutrition, medication adherence, disease management, and preventive care strategies.
Collaborate with physicians, specialists (e.g., gastroenterology, ophthalmology), and allied health professionals to coordinate comprehensive care plans.
Assist in or conduct procedures consistent with the practices specialty and the individuals skills, training and credentials.
Participate in clinical research initiatives or quality improvement projects to enhance patient outcomes and uphold NCQA standards.
Ensure compliance with HIPAA regulations by maintaining accurate medical documentation and safeguarding patient confidentiality.
Maintain confidentiality of all patient, medical, financial, and legal information.
Report to work on time and as scheduled.
Wear identification during work hours.
Represent the organization in a positive & professional manner.
Ability to handle difficult situations involving patients, physicians, or others in a professional manner.
Ability to work independently, exercise creativity, be attentive to detail, and maintain a positive attitude.
Comply with all organizational policies and standards regarding ethical business practices.
Participate in performance improvement and continuous quality improvement activities.
Attend regular staff meetings and in-services as directed by your manager.
Consistently demonstrate the value of the team concept.
Ability to manage multiple and simultaneous responsibilities.
Complete assignments efficiently and in a timely manner.
Valid Nurse Practitioner licensure with current certification in your practicing state; additional certifications in internal medicine preferred.
Demonstrated experience working in acute carel environments such as hospitals
Proficiency with EMR/EHR systems including Epic, Cerner or Athenahealth; familiarity with CPT coding and ICD coding (ICD-9/10).
Strong knowledge of physiology, anatomy, pharmacology (including psychopharmacology), and medical terminology.
Experience performing complex procedures such as suturing, phlebotomy, venipuncture, sterile processing techniques or airway management.
Critical care experience including ICU or PICU settings; hospital medicine or emergency medicine background highly valued.
Ability to work effectively with diverse populations including , geriatrics (memory/dementia/dementia care), individuals with disabilities or developmental disabilities.
Knowledge of Workers' Compensation law and post-acute/discharge planning processes; familiarity with infection control protocols is essential.
