Full Job Description

Sub-Acute Resident Care / Unit Manager – Registered Nurse (RN)

Location: Medford Leas, Medford, NJ

Salary: $74,900 - $97,000 per year
Schedule: Full-time | On-site | Mon-Fri 8am - 4:30pm

The Registered Nurse (RN) Unit Manager / Resident Care Manage (RCM) plays a key leadership role in ensuring the delivery of the highest quality, resident-centered care. The RN RCM promotes resident independence, dignity, and well-being through clinical excellence, effective team leadership, and interdisciplinary collaboration.

This registered nurse position oversees a 14-bed sub-acute rehab unit in an assisted living facility, providing hands-on guidance to nursing staff and ensuring regulatory compliance and clinical best practices.

Key Responsibilities

Maintain accountability for unit operations
Assess, monitor, and coordinate individualized care plans for residents.
Supervise, mentor, and evaluate nursing staff performance.
Ensure compliance with all state and federal regulations and uphold resident rights.
Collaborate with physicians, interdisciplinary teams, and families to optimize care outcomes.
Participate in quality improvement, infection control, and staff development initiatives.
Collaborates with RNAC/MDS coordinator to establish individual care plans based on the Care Area Assessment triggers from the Minimum Data Set ( MDS)

New Jersey Registered Nurse RN License required

Current New Jersey Registered Nurse RN License (required).
Bachelor of Science in Nursing (BSN) preferred.
2–5 years of experience in sub-acute care.
Proven leadership experience in registered nurse management, including staff development and performance management.
Strong understanding of state and federal long-term care regulations and compliance practices.
CPR and AED certification required.
Experience with MDS preferred.
