Position Summary
The Clinical Liaison serves as a key representative of the skilled nursing facility, responsible for developing and maintaining strong relationships with hospitals, physicians, case managers, and community partners to support admissions growth. This role facilitates the clinical review of potential referrals to ensure appropriate placement and acts as a backup to the Director of Admissions, ensuring continuity of the admissions process.
Key Responsibilities
Hospital & Community Outreach:
- Develop and maintain relationships with hospital discharge planners, case managers, social workers, and physicians.
- Conduct routine hospital visits to generate referrals and promote facility services.
- Serve as the primary point of contact for assigned referral sources.
- Represent the facility at community events and networking opportunities.
Clinical Review & Admissions Coordination:
- Review incoming referrals for clinical appropriateness, payer source, and service capability.
- Collaborate with nursing, therapy, and interdisciplinary teams to determine acceptance.
- Communicate admission decisions promptly to referral sources.
- Coordinate with internal departments to ensure smooth and timely admissions.
Admissions Support (Backup Role):
- Act as backup to the Director of Admissions when needed.
- Manage inquiry calls, referrals, and admission documentation.
- Assist with bed management, census tracking, and admission projections.
- Ensure all admission paperwork is completed accurately and timely.
Customer Service & Family Communication:
- Serve as a liaison between families, residents, and the facility during the admission process.
- Provide facility tours when needed.
- Address questions related to services, payer sources, and expectations.
Documentation & Compliance:
- Maintain accurate referral logs and admission tracking reports.
- Ensure compliance with all regulatory requirements, including HIPAA.
- Assist with data tracking related to census, referral sources, and conversion rates.
Qualifications
Education:
- Unrestricted Virginia Licensed Nurse (LPN or RN) preferred OR
- Bachelor’s degree in healthcare, marketing, or related field (with relevant experience)
Experience:
- 2–3 years of experience in skilled nursing, hospital discharge planning, case management, or healthcare marketing.
- Prior admissions or liaison experience preferred.
- Knowledge of Medicare, Medicaid, managed care, and PDPM preferred.
Skills & Competencies:
- Strong clinical assessment skills (if licensed)
- Excellent communication and relationship-building skills
- Ability to multitask and manage a fast-paced environment
- Strong organizational and problem-solving abilities
- Knowledge of EMR systems and Microsoft Office
Physical Requirements
- Frequent travel to hospitals and community partners
- Ability to stand, walk, and sit for extended periods
- Ability to lift up to 25 pounds (if needed)
Additional Expectations
- Participate in on-call rotation for admissions as needed
- Support facility census goals and strategic initiatives
- Maintain a professional image representing the organization
Compensation & Incentives
The Clinical Liaison is eligible for quarterly bonuses based on individual performance and overall facility census growth. Bonus structure will be tied to key performance indicators including referral generation, conversion rates, and contribution to census development.
