Full Job Description

GENERAL DESCRIPTION

The Program Manager/Case Manager coordinates daily delivery of assigned social service programs, combining direct client service with responsibility for documentation, service coordination, and compliance with organizational policies and funding requirements. Grant writing, budgeting, and board or funder reporting remain centralized with executive leadership. Staff are expected to understand program parameters and escalate issues appropriately.

JOB RESPONSIBILITIES

Core Responsibilities

Program Delivery

· Conduct intake, eligibility verification, service planning, documentation, and follow-up in accordance with CALC policies and funding requirements.

· Maintain accurate and timely client records in required data systems.

· Identify barriers in delivering core responsibilities and escalate concerns timely.

Client Coordination

· Advocate for clients within program guidelines.

· Coordinate services with internal teams, programs and community partners.

· Communicate professionally with internal teams, clients, vendors, and partners.

Operational Accountability

· Track and report program activity and meet documentation deadlines.

· Notify leadership of compliance, safety, or service continuity concerns.

· Participate in audits, monitoring, and program reviews.

· Facilities and Resources (as applicable)

· Initiate maintenance requests and report safety concerns.

· Monitor waitlists and housing readiness.

· Ensure appropriate use of program resources.

Professional Collaboration

· Maintain professional boundaries.

· Participate in meetings, trainings, and team collaboration.

· Operates as a member of the CALC team, assigned to the Program Management/Case Management team, providing support, collaboration and assistance as needed.

Scope of Authority

· Program Managers/Case Managers may independently:

· Make routine decisions within established policy

· Coordinate services within program guidelines

· Communicate with clients and approved partners

· Make program-approved purchases within established spending guidance to support effective service delivery

· Program Managers/Case Managers must escalate:

· Policy deviations or eligibility disputes

· Compliance, safety, or spending concerns outside established guidance

· Unusual client circumstances

· Matters involving funders, auditors, board, or media

EDUCATION/SKILL REQUIREMENTS

· Bachelor’s Degree in social work, psychology, sociology, or human services.

2 years of case management experience.

· Strong organization, communication, and documentation skills

· Ability to manage competing priorities independently

OTHER REQUIREMENTS

· Pass criminal background check and drug screening.

· Must pass all drug and alcohol screening, including pre-employment testing and random drug screens, in accordance with CALC's drug and alcohol-free environment policies.

· Must have reliable transportation for job-related travel.

· Must possess a valid state driver's license and have a clean driving history.

· Must be willing to submit to yearly TB testing or provide exemption documentation.

· Maintain client confidentiality per CALC and HIPAA regulations.

· Ability to maintain professional boundaries and adhere to policies and procedures.

· Strong written and verbal communication skills.

· Ability to assess problems, collect data, and develop logical solutions.

Job Type: Full-time

Pay: $22.00 - $24.00 per hour
