Full Job Description

The Case Manager plays a key role in supporting participants enrolled in GWTP’s workforce development, education, and social service programs. This position is responsible for providing high-quality case management, determining eligibility, coordinating supportive services, and monitoring participant progress. The Case Manager collaborates closely with internal teams, external partners, and program participants to ensure successful outcomes and full compliance with grant requirements. This includes maintaining accurate documentation, attending contract meetings, and supporting program activities that promote participant success.

Participant Engagement & Enrollment

● Identify and enroll participants through outreach, community partnerships, and events.

● Conduct program orientations to explain services, expectations, and grant requirements.

● Review applications and documentation to determine eligibility and ensure compliance with funding guidelines.

Case Management & Support Services

● Conduct assessments to identify participant needs and barriers.

● Develop individualized service plans and maintain regular check-ins to support progress. ● Connect participants to training, education, employment opportunities, and supportive services.

● Coordinate resources such as transportation assistance, childcare, work attire, and other assistance.

Data Management & Compliance

● Maintain accurate case notes and enter participant data into Salesforce and other grant databases.

● Ensure all case files are complete, current, and compliant with grant and audit requirements.

● Assist with grant reporting, outcome tracking, and adherence to funding guidelines.

● Stay informed on program requirements and communicate updates to the team.

Program & Team Support

● Support program activities including Job Readiness, Life Skills, Digital Literacy, Senior Portfolio, and Financial Literacy classes.

● Assist with event coordination and tracking of grant deliverables.

● Prepare for internal and external audits as needed.

● Attend contractor meetings and training to stay aligned with program expectations. ● Collaborate with Program Managers, instructors, and support staff to promote participant success.

Other Duties

● Perform additional responsibilities as assigned to support program goals and organizational needs.

Qualifications/Requirements:

● Bachelor’s degree in social work, human services, education, or a related field (or 5 years of equivalent experience).

● Experience in case management, workforce development, or social services preferred.

● Strong organizational skills, attention to detail, and ability to manage multiple tasks.

● Comfortable working with diverse populations and addressing barriers to employment and education.

● Proficiency in Microsoft Office and case management databases.

● Strong communication and teamwork skills.
