The QIDP serves as an integral member of the organization’s Community Day Services Program, Home-Based Services Program, and In-Home Respite Program. The QIDP collaborates with direct support professionals, respite workers, home-based service providers, case managers, families, and community partners to ensure the provision of high-quality, person-centered support across all three programs. The QIDP is responsible for individualized service planning, ongoing oversight of service delivery, supervision of direct service staff across assigned programs, documentation and compliance, and IDHS billing functions. The QIDP also supports the Director of Community Programs with quality assurance, program monitoring, and regulatory compliance.
Primary Functions and Accountabilities:
· Oversee the implementation and coordination of services within the Community Day Services Program, the Home-Based Services Program, and the In-Home Respite Program.
· Support development, execution, and updates of program policies, procedures, and protocols to ensure compliance with IDHS regulations, agency requirements, and applicable licensing standards.
· Develop, implement, evaluate, and revise implantation strategies for participants in each assigned program.
· Collaborate with participants, families, and multidisciplinary teams to ensure services reflect participant preferences, goals, and rights.
· Monitor and review the implementation of service plans, making necessary adjustments to ensure optimal outcomes.
· Complete IDHS billing for assigned programs, ensuring compliance with billing requirements and utilizing relevant billing systems efficiently and accurately.
· Maintain current knowledge of IDHS billing codes and procedures, attending required training sessions.
· Conduct regular audits of service documentation and billing records to ensure accuracy, completeness, and adherence to IDHS guidelines.
· Maintain accurate and up-to-date records, documenting participant interactions, progress, and service authorizations.
· Prepare reports, statistical data, and other documentation required by regulatory agencies, funding sources, and organizational leadership.
· Provide guidance, training, and supervision to direct service staff regarding program procedures, documentation, and billing practices.
· Participate in the recruitment, hiring, onboarding, and ongoing supervision of direct service staff.

· Support the Director of Community Programs with monitoring preparation, audit readiness, and corrective action.
Community Day Services Program Responsibilities:
· Ensure compliance with all Division of Developmental Disabilities (DDD) and Bureau of Quality Management (BQM) requirements applicable to Community Day Services.
· Conduct quarterly file reviews and update documentation in accordance with IDHS and Medicaid Waiver requirements.
· Oversee ROCS billing for Community Day Services, ensuring accurate service entries and timely submissions.
· Coordinate with Independent Service Coordination (ISC) agencies for individual service planning and updates.
· Monitor staff-to-participant ratios, service delivery quality, and program safety daily.
· Lead Personal Plan meetings and ensure participation by the individual, family or guardian, and relevant team members.
· Review and approve daily documentation and incident reports completed by Direct Support Professionals.
· Ensure compliance with Office of Inspector General (OIG) Rule 50, abuse and neglect reporting, and human rights protections specific to adult day programming.
· Support program activities that promote community integration, skill development, and personal outcomes.
Home-Based Services Program Responsibilities:
· Develop and maintain Personal Plans and Implementation Strategies for each individual enrolled in the Home-Based Services Program in accordance with IDHS requirements.
· Coordinate with families, ISC agencies, and approved service providers to develop monthly service budgets that align with the individual’s authorized funding and goals.
· Authorize and monitor purchases made through the Home-Based Services budget, ensuring services are allowable, person-centered, and consistent with the Personal Plan.
· Conduct monthly contacts with each enrolled individual and family to review service utilization, address concerns, and adjust the Personal Plan as needed.
· Complete and submit IDHS Home-Based Services billing and reimbursement documentation accurately and on time.
· Ensure all Home-Based Services providers meet program qualifications and complete required documentation prior to service delivery.
· Review provider invoices, receipts, and service documentation for accuracy and compliance with IDHS guidelines.
· Conduct annual reassessments of individual needs, goals, and budget allocations.
· Maintain compliance with IDHS Home and Community-Based Services (HCBS) waiver requirements applicable to Home-Based Services.
· Provide families with information, referrals, and support to access community resources that complement Home-Based Services.
In-Home Respite Program Responsibilities:
· Conduct intake, eligibility verification, and Needs Assessments for families referred to the In-Home Respite Program.
· Develop and maintain the Individualized Respite Support Plan for each enrolled participant in collaboration with the family.
· Verify HCBS waiver non-enrollment at intake and on an annual basis.
· Match families with appropriately trained respite workers based on assessed needs, family preferences, and worker availability.
· Provide direct supervision and ongoing oversight of all assigned respite workers, including monthly check-ins and documentation review.
· Review Paylocity time records, Microsoft Forms contact notes, and caregiver verification for each service period and resolve discrepancies prior to payroll processing.
· Conduct monthly documentation spot-checks (minimum 10% of active participant files) and quarterly comprehensive file reviews.
· Administer quarterly family satisfaction surveys and implement corrective action when results fall below program standards.
· Prepare quarterly performance reports for IDHS, including utilization, demographic, and outcome data.
· Respond to emergency and crisis respite requests, coordinating expedited service initiation when needed.
· Ensure compliance with the Illinois Mental Health and Developmental Disabilities Confidentiality Act (740 ILCS 110) and HIPAA in all interactions involving participant information.
MINIMUM QUALIFICATIONS:
- Education/Certification
- Bachelor's degree in Social Work, Psychology, Special Education, or a related field.
- QIDP certification from IDHS or the ability to obtain certification within six months of employment.
- Experience
- At least two years of experience working with individuals with intellectual disabilities and/or developmental disabilities.

Work Environment: The QIDP will split their time between a community day services program, in-home respite program, and a home-based services program. This role requires flexible working hours and occasional travel to different program locations within the geographical area.
