Full Job Description

Overview
We are seeking a dedicated and organized Care Coordinator to join our team. This is a full-time on-site role based in Orland Park, IL. The ideal candidate will play a crucial role in ensuring that our clients receive the highest quality of care by effectively managing their healthcare needs.

Responsibilities:

Lead and manage day to day care coordination tasks including planning, assessing and facilitating client care.
Must be able to manage client case management, coordinating home care services, handling client inquiries with exceptional customer service, and communicating effectively with clients, their families, and care providers.
Scheduling Caregivers with Clients and Task Creation based on clients' needs
Ensure IDPH Compliance and appropriate documentation

Experience Required:

Previous experience in senior home care required
Passion and motivation to work with seniors and their families
Strong organizational skills with the ability to manage multiple tasks simultaneously.
Excellent interpersonal skills to foster positive relationships with clients, family members and community staff.
Proficiency in office software applications to maintain records and manage scheduling efficiently.
