Full Job Description

This is an excellent part-time opportunity for an experienced Registered Nurse to join a growing team of outstanding geriatric care managers in the greater Chicagoland area. Care Managers must drive and be willing to travel throughout the Chicago area and western suburbs. This position pays $55.00 per hour. All client-related travel is reimbursed at $25.00 per hour.

Responsibilities

· Conduct initial assessments of new clients to determine current medical, emotional, and social needs.

· Perform comprehensive in-home assessments for prospective Home Companion Solutions clients (our sister company) to determine appropriate levels of care and develop personalized care recommendations.

· Conduct annual supervisory visits for Home Companion Solutions clients to evaluate changes in condition, ensure care plans remain appropriate, and recommend adjustments to services as needed.

· Develop a rapport with clients to become a trusted care provider.

· Provide critical care management and coordination for clients in various healthcare settings, including emergency rooms, behavioral health units, ambulatory clinics, physician offices, and intensive care units.

· Conduct comprehensive assessments of clients' medical conditions, mental health status, and social support systems.

· Develop individualized care plans in collaboration with interdisciplinary teams to address clients' physical, emotional, and psychosocial needs.

· Coordinate and facilitate communication between healthcare providers, clients, and their families to ensure continuity of care.

· Monitor clients' progress and adjust care plans as needed to optimize outcomes.

· Educate clients and their families on health management strategies, treatment options, and available community resources.

· Advocate for clients' rights and ensure they receive appropriate medical interventions and support services.

· Document all client interactions, assessments, interventions, and outcomes accurately and timely in the client tracking database.

Skills

· The ideal candidate will be an RN who is bright, organized, can creatively problem solve, is personable, and is a self-starter. Outstanding oral and written communication skills are a must.

· Strong clinical knowledge and experience in geriatrics is preferred.

· Experience conducting in-home assessments and developing person-centered care plans is highly desirable.

· Excellent assessment skills to identify clients' physical and mental health needs accurately.

· Effective communication skills to collaborate with interdisciplinary teams, clients, and their families.

· Ability to prioritize tasks and manage time efficiently in a fast-paced healthcare environment.

· Empathy and compassion to provide emotional support to clients during challenging times.

· Proficiency in using electronic health records and client management systems to document client information.
