Home Health Aide (HHA/CNA) – Part-Time / PRN

Downtown Chicago & North Shore Suburbs (Skokie, Niles, Wilmette, Winnetka)

Responsibilities

As a Home Health Aide/CNA, you will:

Provide personalized care and assistance in patients' homes
Assist with activities of daily living, including bathing, dressing, grooming, and mobility
Support patient comfort, safety, and independence
Observe and report changes in a patient's condition to the clinical team
Assist with basic health-related tasks, including medication reminders and approved personal care services
Follow individualized care plans and agency protocols
Educate and support family members and caregivers as appropriate
Maintain accurate communication with the care team
Ensure a safe, clean, and respectful care environment
Protect patient privacy and confidentiality at all times

Qualifications

We're looking for caregivers who are passionate about helping others and committed to delivering exceptional care.

Requirements:

Active Illinois CNA certification (or qualified Home Health Aide credential)
Previous home health, hospice, private duty, or caregiving experience preferred
Compassionate, patient-focused approach to care
Reliable, professional, and dependable work ethic
Strong interpersonal and communication skills
Ability to perform the physical duties associated with patient care
Valid driver's license and reliable transportation preferred
