Salary: $24 - $46 per hour dependent on qualifications and experience

Position Overview:

My Viva Plan Inc. is seeking a highly organized, compassionate, and detail-oriented CMS ACCESS Care Coordinator to support Medicare beneficiaries participating in the Centers for Medicare & Medicaid Services (CMS) ACCESS Model. The Care Coordinator serves as the primary point of contact for participants enrolled in the program, ensuring timely communication, care coordination, patient engagement, documentation, and collaboration with providers, lifestyle coaches, and interdisciplinary care teams. This position plays a critical role in improving beneficiary outcomes by coordinating services, supporting chronic disease management, facilitating communication between healthcare providers, and ensuring compliance with CMS ACCESS Model requirements.

Patient Care Coordination

Coordinate services for Medicare beneficiaries enrolled in the CMS ACCESS Model.
Conduct patient outreach via telephone, secure messaging, and virtual platforms.
Schedule follow-up appointments with providers, lifestyle coaches, and specialists.
Monitor patient participation and engagement throughout the program.
Assist beneficiaries in understanding their individualized care plans.
Identify barriers to care and coordinate appropriate interventions.

Care Management

Review patient assessments and care plans.
Monitor care updates submitted through the ACCESS platform.
Coordinate preventive care services.
Support patients managing:
Cardio-Kidney-Metabolic (CKM)
Early CKM (eCKM)
Musculoskeletal (MSK)
Behavioral Health (BH)
Assist patients with referrals and community resource connections.

Documentation & Compliance

Document all patient interactions accurately within the designated electronic health record (EHR) and care management platform.
Maintain HIPAA compliance and patient confidentiality.
Ensure documentation supports CMS ACCESS Model requirements.
Track required quality measures and programming milestones.
Assist with audit readiness and documentation reviews.

Quality Improvement

Monitor patient outcomes.
Track patient engagement metrics.
Assist in quality improvement initiatives.

Recommend workflow improvements.
Associate’s degree required.
Bachelor's Degree preferred.
Minimum of two (2) years of healthcare coordination or care management experience.
Experience working with Medicare populations preferred.
Strong organizational and communication skills.
Excellent customer service skills.
Ability to manage multiple priorities.
Proficient in Microsoft Office 365.
Comfortable learning new healthcare technologies.
