Full Job Description

Job Title/Position: Hospice Chaplain/Spiritual Adviser Reports To: FLSA Status: Director of Clinical Services Exempt JOB DESCRIPTION SUMMARY The Hospice Chaplain/Spiritual Adviser is responsible for the provision of spiritual care services to patients and families/caregivers of ECHO Hospice, either directly or through coordination of care with other spiritual counselors. Responsible for implementing and coordinating all activities of the spiritual services programs and the delivery of spiritual services to patients/families. Provide direct spiritual support and/or counsel to patients/families while keeping with the patient's/family's spiritual beliefs. Work with staff, clergy and community groups to enhance sensitivity to the spiritual concerns’ patient and family. Participate in IDT to address spiritual concerns. Provide scheduled visits and "as needed" visits to address spiritual concerns. Provide bereavement visits as required. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES 1. Assures spiritual assessment of patients and families/caregivers in the hospice program and appropriate services are rendered in a timely manner. 2. Facilitates the development of the individualized the plan of care by participating in the comprehensive assessment to meet identified spiritual needs. 3. Provides direct spiritual care to patients and families/caregivers. 4. Serves as liaison and support to community chaplains and spiritual counselors. 5. Maintains records of spiritual care services utilization and related activities for quality assurance, program development, and policies and procedures review and revision. 6. Documents direct services and ongoing communication with community chaplains and spiritual counselors. 7. Attends patient care conference and interdisciplinary group meetings as a member of the interdisciplinary group. 8. Provides consultation, education, and support to the interdisciplinary group on spiritual care. 9. Recruits community chaplains, spiritual counselors, and spiritual care volunteers adequate to meet patient and family/caregiver needs by developing community contacts and offering education through congregations about hospice care. 10. Provides for funeral or memorial services for patients as requested. Chaplain JD 06.20.25 1 11. Plans periodic memorial services to meet the needs of personnel, volunteers, and community clergy/spiritual counselors working with the hospice team. 12. Assists in supervision of spiritual care volunteers when assigned to patients/ families/caregivers. 13. Actively participates in quality assessment performance improvement teams and activities. 14. Accepts and performs other related duties and responsibilities as required. 15. Ability to cope with patient/family emotional stress and tolerance for individual lifestyles. POSITION QUALIFICATIONS 1. Graduate of accredited seminary or school of theology or have appropriate certification of pastoral ministry. Experience working with death and dying, individuals/family/caregiver. 2. Ability to work as member of interdisciplinary group. 3. Comfort in an interfaith setting. 4. Ability to accept different lifestyles, cultures, beliefs, and values. 5. Ability to network with community clergy and congregations. 6. Knowledge of and commitment to hospice philosophy of care. 7. Hospice/ experience preferred. 8. Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order. 9. Basic knowledge of local, state, and federal hospice regulations. 10. Practices of spiritual counseling/advising. 11. Death and dying process. 12. Strong communication skills. 13. Ability to deal with a variety of personalities and complex situations. 14. HIPAA regulations and other local, state, and federal patient information regulations. PHYSICAL REQUIREMENTS: 1. The ability to sit frequently for 3-6 hours. 2. The ability to frequently stand for 3-6 hours. 3. The ability to occasionally drive/ operate foot control for 1-3 hours. 4. The ability to walk intermittently on level and or unlevel surface for less than 1 hour. 5. The ability to intermittently bend or climb for less than 1 hour. 6. The ability to occasionally twist, kneel, crouch and crawl for 1-3 hours. 7. The ability to lift 10 pounds occasionally for 1-3 hours. 8. The ability to lift 11-50 pounds intermittently for less than 1 hour. 9. The ability to carry up to 10 pounds occasionally for 1-3 hours. 10. The ability to carry 11-50 pounds intermittently for less than 1 hour. 11. The ability to push up to 10 pounds occasionally for 1-3 hours. 12. The ability to push 11-50 pounds intermittently for less than 1 hour. 13. The ability to pull up to 10 pounds occasionally for 1-3 hours.

Pay: $68,000.00 - $75,000.00 per year
