Remote Clinical Pharmacist (Medication Therapy Management)

Position Type: Full-Time / Direct Hire

Location: Fully Remote (US-based)

Pay Rate: $57.00 - $65.00 / hour (Dependent on experience)

Shift: Monday - Friday, 8-hour shift between 6:00 AM - 7:00 PM PST (Greater need for mid-morning to late morning PST start times)

Position Overview

Are you a compassionate Clinical Pharmacist who thrives on direct patient interaction and complex medication management? We are seeking a self-motivated, tech-savvy clinician to join a growing clinical care team.

In this patient-facing, phone-based role, you will practice at the top of your license-delivering high-touch medication therapy management, conducting comprehensive medication reviews, and working directly with complex patients, providers, and pharmacies to improve healthcare outcomes and eliminate avoidable harm.

Key Responsibilities

Patient Engagement & Outreach: Conduct outbound calls (40+ per day) to engage complex patients, conduct Comprehensive Medication Reviews (CMRs), Targeted Medication Reviews (TMRs), and provide medication adherence counseling.
Clinical Consultations: Review complex patient medication profiles, evaluate therapeutic appropriateness across chronic disease states, and recommend interventions to optimize drug therapy and lower hospitalization risks.
Stakeholder Collaboration: Frequently contact provider offices, community pharmacies, and clinics to resolve therapy barriers, verify prescriptions, and handle care transitions.
Platform & Workflow Usage: Document clinical interactions, patient data, and care plans within proprietary cloud software while utilizing cross-functional tools (e.g., Slack, Five9).
Metrics & Quality: Meet daily standards for productivity, documentation quality, and engagement while adapting to evolving clinical projects and health plan needs.

Qualifications & Requirements

Required:

Education: Doctor of Pharmacy (PharmD) from an accredited university.
Licensure: Active Pharmacist license in good standing (holding licenses in multiple states is a major plus).
Clinical Knowledge: Strong understanding of CMS quality measures, Star Ratings, HEDIS metrics, and Medicare Part C/D measures across complex chronic diseases.
Communication Skills: Exceptional phone presence, motivational interviewing skills, and the ability to confidently cold-call and build trust with patients, pharmacies, and doctors.
Workstyle Fit: Thrives in a high-volume, metrics-driven phone environment, demonstrates independence, and takes coaching constructive feedback well.
Tech Proficiency: Comfortable navigating Apple/Mac OS, Google Workspace, multi-browser setups, contact center software, and high-speed typing.
Remote Setup: Designated, quiet, private home office with high-speed internet capability.

Preferred Skills & Backgrounds:

Bilingual Skills: Fluency in Spanish (highly preferred), Cantonese, Mandarin, or Russian.
Practice Background: Experience in Ambulatory Care, Managed Care, Health Plans/PBMs, Hospital/Emergency Care, or High-Volume Community Pharmacy.
Certifications/Residency: Board certification or PGY1/PGY2 residency (Ambulatory Care, Managed Care, or Community-based).
MTM Software: Prior experience utilizing medication therapy management software platforms.
