Position Summary:
The Case Manager provides structured support and guidance to a caseload of men, women and families residing at the Torres Community Shelter. The Shelter serves a diverse population of at-risk individuals who may be experiencing mental illness, developmental and/or physical disability, substance dependence, domestic violence, recovery and who may have legal issues involving probation and parole. Case plans are designed, implemented and continually revisited to address barriers to self-sufficiency and to encourage and support guests in securing appropriate public social services benefits and/or employment, mental and physical health care, appropriate housing and a variety of other individualized services.
Education and/or Experience:
· A.A. degree or higher in social and behavioral sciences, or closely related field of study preferred and/or 2 years of experience working in a crisis setting or closely related field.
· Excellent crisis intervention skills and experience required.
· Previous experience working with the homeless population is preferred.

· Knowledge of housing resources including Rapid Rehousing Funds, TBRA, Section-8, VASH and local opportunities for temporary and permanent supportive housing.
· Thorough understanding of mandated reporting laws for children, the elderly and those with disabilities.
· Knowledge of HIPPA laws and other standard practices for protecting guest information and maintaining confidentiality.
· Ability to effectively and efficiently gather, organize and document information.
· Ability to make appropriate and meaningful referrals and advocate for guests.
· Interviewing skills in the area of identifying mental illness or other behavioral issues or circumstances which require supplemental intervention and support.
· Crisis management skills to quickly assess the level of intervention needed and to actively seek necessary supports in instances of health and safety emergencies.
· Professional demeanor and positive attitude reflecting the core values of the Shelter including compassion, accountability and dignity for all guests.
· Ability to work under stress in an environment where your clients reside.
· Knowledge of universal precautions and understanding of what conditions constitute a medical or situational emergency.

Responsibilities:
· Along with the other Torres Case Managers, ensure that every guest has a current case plan that is in alignment with their current needs and goals and ensure that regular case plan meetings occur with each guest.
· Assist individuals and families in obtaining permanent housing including facilitating enrollment in CCSP Rapid Rehousing Program, assisting guests in applying for Section 8, TBRA, VASH, Esplanade House, Sabbath House and other housing options.
· Coordinate case plan meetings with guests and maintain accurate calendar of appointments.
· Document all interactions with guests as well as communication with other agencies on guest’s behalf.
· Assess guest needs and strengths and, in collaboration with guest, incorporate these into the case plan and make appropriate referrals; follow up on referrals to ensure guest is truly connected.
· Use ‘Torres Case Management Tools’ to assist guests in meeting their goals.
· Maintain the file of each guest; in addition to case notes, plans and other Torres specific paperwork and tools, ensure copies of all important documents are organized within the file.
· Conduct Exit Interviews for all guests leaving the shelter.
· Facilitate weekly ‘Guest Orientation’ for new arrivals.
· Maintain daily log to ensure all staff are informed of applicable guest situations/circumstances; document all ‘passes’.
· Ensure that guest information in HMIS – CLARITY is accurate and current; assist with reporting requirements.
· Professionally and effectively interact with a diverse population of guests, staff, volunteers, donors and the board members as well as outside agencies including partner agencies and emergency personnel.
· Report all instances of abusive, suspicious, and/or criminal behavior to the proper authorities.
· File mandated child and elder abuse and neglect reports as necessary; communicate with supervisor.
· Participate in problem solving and organizational planning with the leadership team.
· Solve problems independently and promptly while maintaining open lines of communication with supervisor; seek consultation as needed.
· Attend and actively participate in monthly all-staff meetings, trainings, leadership meetings, individual and group consultations, and meetings with other agencies and collaborative partners.
· Represent the Shelter at meetings with other agencies and conduct in-service presentations.
· Be able to lift and carry 35 pounds from an overhead position to the floor.
· Travel for work-related trainings and Shelter errands.
· Adhere to all organizational policies and procedures.
· Complete the above and other responsibilities as assigned in a timely manner.
Rapid-Rehousing (RRH) Case Manager Specialty
Management of ‘Case Manager’ duties and the following additional responsibilities:
· Monthly individual contact (phone or preferably physical) with all RRH clients in non-CHAT housing
· Monthly group house meetings for clients residing in CHAT housing
· Maintain accurate records and files including case notes for each client contact
· Complete and maintain documentation requirements for RRH
· Maintain RRH housing list (including rent, openings, and current placements)
· Ensure that each client complies with lease agreement
· Intervene as necessary to address issues of compliance
· Monitor and ensure cleanliness, health and safety of client residence with guidance and referrals as necessary
· Collect rent from all clients monthly
· Participate in weekly CHAT team meetings
· Work closely with CHAT team to ensure housing maintenance, timely communication regarding housing availability, and current client lease compliance
· Work closely with Executive Director and communicate frequently regarding all RRH progress and concerns
Families Case Manager Specialty
Management of ‘Case Manager’ duties and the following additional responsibilities:
· Monitor attendance to parenting classes with Parenting with Pride though the Child Abuse Prevention Council Earn While You Learn through the Women’s Resource Clinic
· Coordinate attendance and transportation with Justin Margolis and the Parent Café for monthly events
· Coordinate and maintain open communication case workers for all families with all open CSD cases
o Fax ROI with coversheet, communicate any dirty drug tests, communicate absences, positive progress feedback and attend CSD meetings if invited by guest
· Coordinate family specific donations and distribution of goods
· Manage family room placements, daily cleaning check of each family room and resulting coachings, and reporting of any maintenance needs, communication via the log to all staff of any concerns
· Manage E-Center (Head Start and Early Head Start) applications – Lily Becerra
· Manage Esplanade House applications - Diane
· Coordinate with Counseling Solutions (Children’s Assistance Program C.A.P. through CSD) - Janet
· Work closely with Mental Health Specialist in monitoring and consultation – Mandi
· Coordinate with Far Northern and Turning Point – FNRC High Needs Case Manager - Morgan Curtis
· Coordinate with Butte County’s CalWORKs Housing Support Program for Families – Deanna Feliciano
· Communicate with Welfare-to-Work case managers as needed – Alise Pugh, Senior Case Manager
