ABOUT THE POSITION
The Housing Case Manager provides structured support and guidance to a caseload of men, women and families enrolled in True North Housing Alliance (TNHA) programs. TNHA serves a diverse population of at-risk individuals who may be experiencing mental illness, developmental and/or physical disability, substance dependence, domestic violence, recovery and who may have legal issues involving probation and parole. Case plans are designed, implemented and continually revisited to address barriers to self-sufficiency and to encourage and support clients in securing appropriate public social services benefits and/or employment, mental and physical health care, appropriate housing and a variety of other individualized services.
DUTIES AND RESPONSIBILITIES
· Ensure every client has a current case plan aligned with their needs and goals.
· Conduct twice monthly case plan meetings with each client.
· Assess client needs and strengths; incorporate these into the case plan.
· Make appropriate referrals and follow up to ensure connection.
· Use TNHA Case Management Tools to assist clients in meeting their goals.
· Interact professionally with a diverse population of clients, staff, volunteers, donors, board members, and outside agencies.
· Report instances of abusive, suspicious, and/or criminal behavior to appropriate authorities.
· File mandated child and elder abuse reports; communicate with supervisor.
· Participate in problem-solving and organizational planning with the leadership team.
· Attend and actively participate in monthly all-staff meetings, trainings, and meetings with other agencies.
· Provide support to clients in sustaining housing
· Coordinates with Housing Navigator and provides housing mediation and conflict resolution services with landlords and property managers.
· Represent TNHA at meetings with other agencies.
· Adhere to all organizational policies and procedures.
· Complete assigned responsibilities in a timely manner.
· Maintain effective communication with service providers and external partners.
· Ensure clients develop and maintain essential life skills for housing stability.
· Provide continuous support and coaching to clients in managing their housing; support and engage clients in socialization activities.
· Collaborate with external agencies to enhance housing support services.
· Monitor client progress and adjust case plans as needed.
· Address and resolve any issues related to housing compliance and lease agreements, facilitate landlord mediation services.
· Ensure accurate documentation of client interactions and case notes.
· Maintain effective communication with service providers and external partners.
· Represent the organization at meetings and in-service presentations related to housing support.
· Read and update the logs daily.
· Participate in Biweekly 1:1 with Case Manager Administrator
· Conduct reconsideration appointments weekly
· Meet in greet with new intakes daily after orientation
· Participate in Weekly Group consultation
· Regularly review and update housing support plans.
· Provide referrals to housing navigation and service coordination.
· Provide referrals to Cal-Aim
· Assist and refer clients applying for Social Security benefits
· Assist Clients with obtaining important documents
· Coordinate case plan meetings with clients.
· Maintain an accurate calendar of appointments.
· Document all interactions with clients and communication with other agencies on them in HMIS
· Maintain organized files for each client, including case notes, plans, and important document in HMIS
· Advocate for clients with outside agency
· Upload all client documents into HMIS
· Ensure client information in HMIS – CLARITY is accurate and current.
· Conduct exit interviews for all clients leaving the shelter.
· Conduct Exit survey for all clients entering into permanent/transitional housing.
· Maintain case management trackers; document all passes and client accommodation
· Travel for work-related trainings and shelter errands.
· Attend 1:1 weekly meetings with Housing Service Administrator.
· Develop and regularly update individualized housing stability plans.
· Provide life skills training, including budgeting, housekeeping, and conflict resolution.
· Coordinate and facilitate workshops and classes focused on life skills.
· Assist clients in applying for housing programs (Section 8, TBRA, VASH as needed.
· Facilitate enrollment in housing support programs (CCSP Rapid Rehousing).
· Monitor client compliance with lease agreements and provide support as needed.
· Maintain accurate and up-to-date records in HMIS – CLARITY.
· Prepare and submit required reports on client progress and outcomes.
· Provide continuous support and coaching to clients in managing their housing.
· Collaborate with external agencies to enhance housing support services.
· Performs other duties as assigned.
QUALIFICATIONS
· High School Diploma required.
· Associates or Bachelor’s Degree in Social and Behavioral Sciences, or closely related field of study preferred and/or 2 years of experience working in a crisis setting or closely related field.
· Excellent crisis intervention skills and experience required.
· Previous experience working with the homeless population is preferred.
