Full Job Description

Job Summary

We are seeking a dedicated and highly skilled LPN to join our healthcare team on the overnight shift from 7 p.m. to 7 a.m. This full-time position offers an opportunity to provide comprehensive patient care in a dynamic, fast-paced environment. The ideal candidate will possess extensive clinical expertise, demonstrate exceptional patient assessment skills, and be committed to delivering high-quality nursing services across diverse medical settings. You will play a vital role in ensuring patient safety, comfort, and well-being during overnight hours while collaborating with multidisciplinary teams.

Duties

Perform thorough patient assessments, including vital signs, physical examinations, and medical history reviews, to determine care needs during overnight shifts.
Administer medications and injections accurately following established protocols,
Monitor patients' conditions closely using telemetry and other monitoring systems; respond promptly to emergencies such as acute pain episodes or respiratory distress.
Document all patient interactions and interventions accurately within electronic health record (EHR) systems like Epic or Cerner, ensuring compliance with HIPAA regulations.
Assist with complex procedures such as suturing, catheterization, tube feeding management, and ventilator care in accordance with hospital standards.
Collaborate with physicians and specialists in areas such as cardiology, nephrology (dialysis), trauma medicine, or behavioral health to coordinate comprehensive care plans.
Provide specialized care for vulnerable populations including geriatrics, pediatrics, neonatal patients, or individuals with developmental disabilities; support memory care and hospice needs as required.
Maintain infection control standards through proper sterilization techniques and adherence to CDC guidelines; participate in quality improvement initiatives aligned with NCQA standards.

Skills

Proficiency in EMR/EHR systems such as PCC
Strong knowledge of medical terminology, anatomy, physiology, and diagnostic evaluation methods relevant to acute and critical care environments.
Experience working within Long-Term Care facilities
Skilled in advanced nursing procedures including IV insertion, phlebotomy, laboratory specimen collection/processing, and aseptic technique.
Ability to perform complex patient assessments; adept at triage for urgent care situations and managing acute pain or infectious disease cases.
Excellent communication skills for effective collaboration with multidisciplinary teams; capable of providing health coaching and discharge planning support.
Demonstrated commitment to ongoing professional development in areas such as geriatrics, hospice & palliative medicine, or behavioral health nursing.
