Overview
Join our dynamic healthcare team as a Lifestyle Medicine Nurse Practitioner, where you will play a pivotal role in promoting holistic wellness and preventive care. This innovative position combines clinical expertise with health coaching to empower patients to achieve sustainable lifestyle changes. You will work in a collaborative environment dedicated to transforming traditional healthcare into personalized, patient-centered solutions that enhance quality of life and long-term health outcomes.

Duties

Conduct comprehensive patient assessments, including vital signs, medical history, and lifestyle factors, to develop personalized wellness plans.
Provide evidence-based interventions such as nutritional counseling, physical activity guidance, stress management techniques, and behavioral health support.
Administer treatments including botulinum toxin therapy, dermal fillers, esthetic laser procedures, and other non-invasive cosmetic services as appropriate.
Manage chronic conditions related to metabolic syndrome, obesity, hypertension, and diabetes through medication management and lifestyle modifications.
Utilize electronic health record (EHR) systems like Epic or eClinicalWorks for accurate documentation of patient encounters and treatment plans.
Educate patients on disease prevention strategies, medication adherence, and health coaching techniques to foster long-term behavior change.
Collaborate with multidisciplinary teams including physicians, dietitians, physical therapists, and behavioral health specialists to optimize patient care.

Skills

Proven experience working with diverse populations including geriatrics, pediatrics, assisted living residents, hospice & palliative care patients, and individuals with developmental disabilities.
Strong knowledge of clinical procedures such as IV infusion, suturing, catheterization, phlebotomy, sterile processing techniques, and airway management.
Familiarity with medical coding systems including CPT (Current Procedural Terminology), ICD-9/10 (International Classification of Diseases), and ICD coding for accurate billing and documentation.
Expertise in managing acute pain cases, trauma assessments in Level I/II trauma centers or emergency settings, and critical care environments like ICU or PICU.
Experience with telehealth platforms and remote patient monitoring tools to deliver virtual wellness coaching and follow-up care.
Ability to perform diagnostic evaluations using sonography or laboratory testing; knowledge of infection control protocols is essential.
Excellent communication skills for patient education on topics such as nutrition, memory care (including Alzheimer’s/dementia), occupational health, and family planning.
