Full Job Description

The Patient Care Coordinator/Scheduler is responsible for coordinating daily patient visits, managing clinician schedules, supporting admissions, and ensuring timely, efficient delivery of hospice services. This role serves as the communication hub between patients, families, referral sources, nurses, aides, social workers, chaplains, and leadership.

The ideal candidate has prior hospice scheduling experience, exceptional organizational skills, and thrives in a fast-paced environment where priorities frequently change.

Essential Duties & ResponsibilitiesScheduling & Coordination

Coordinate daily schedules for RNs, Hospice Aides, Social Workers, Chaplains, and other clinicians.
Optimize clinician routes to maximize efficiency.
Adjust schedules throughout the day for urgent patient needs.
Schedule admissions, routine visits, PRN visits, deaths, and discharges.
Coordinate weekend and holiday coverage.
Monitor visit completion and address scheduling gaps.

Patient Care Coordination

Serve as the primary communication point for patients and families regarding visit schedules.
Coordinate services with physicians, facilities, and community partners.
Assist with admission logistics.
Track physician orders and ensure timely follow-up.
Monitor patient census and clinician caseloads.

EMR & Documentation

Maintain accurate patient demographic information.
Enter and update scheduling information within the EMR.
Verify visit documentation is completed.
Assist leadership with reports and census tracking.

Team Communication

Communicate schedule changes promptly.
Participate in daily clinical huddles and IDG meetings.
Support field staff by troubleshooting scheduling conflicts.
Collaborate closely with the Clinical Director and Office Manager.

Administrative Responsibilities

Answer incoming calls professionally.
Monitor shared email inboxes.
Process referrals as assigned.
Maintain confidentiality and HIPAA compliance.
Perform additional administrative duties as needed.

QualificationsRequired

Minimum 2 years of hospice scheduling or hospice coordination experience
Knowledge of Medicare hospice regulations
Experience scheduling interdisciplinary hospice teams
Excellent multitasking and organizational skills
Strong customer service and communication abilities
Proficiency with Microsoft Office
Experience with hospice EMR systems

Preferred

Experience with Hospice Tools, MatrixCare, Axxess, or similar hospice software
Experience processing referrals and admissions
Knowledge of hospice eligibility and levels of care

Schedule

Full-Time
Monday-Friday
Typical hours: 8:00 AM-5:00 PM
Occasional flexibility during periods of high census or urgent patient needs.

Pay: $26.00 - $30.00 per hour
