Full Job Description

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) / Licensed Vocational Nurse (LVN) specializing in Long-Term Care (LTC) to join our healthcare team. The ideal candidate will have strong clinical skills and experience providing quality nursing care to residents in skilled nursing facilities, nursing homes, or other long-term care settings. This role requires a commitment to patient-centered care, excellent communication, and the ability to work collaboratively with residents, families, physicians, and nursing staff.

Responsibilities

Provide direct nursing care to residents in accordance with physician orders, care plans, and facility policies.
Monitor residents' physical and mental conditions and promptly report changes in health status.
Administer medications, treatments, and therapies as prescribed and accurately document all care provided.
Perform routine nursing procedures, including wound care, catheter care, vital-sign monitoring, and other clinical tasks.
Assist with oxygen administration and enteral feeding through gastrostomy or jejunostomy tubes as required.
Perform wound care and assist with wound-vac management when applicable.
Receive, review, and carry out physician orders accurately.
Participate in physician rounds and communicate residents' medical needs and changes in condition.
Supervise and support nursing assistants/LNAs and delegate resident-care responsibilities within their scope of practice.
Assist with admissions, discharges, and transfers while completing all required documentation.
Communicate effectively with residents and their families regarding care, concerns, treatment plans, and changes in condition.
Maintain accurate electronic medical records, medication administration records, treatment records, and nursing documentation.
Follow infection-control procedures and ensure a clean and safe environment for residents.
Participate in care planning, quality-improvement activities, staff education, and required facility meetings.
Follow all applicable state and federal regulations related to long-term care and medication management.
Work collaboratively with the nursing supervisor, DON, physicians, therapists, and other members of the interdisciplinary team.
Float to other units when needed, provided the nurse has appropriate experience and competency.

Qualifications

Valid and active LPN/LVN license in the applicable state.
BLS certification required; AHA BLS preferred.
Previous experience in Long-Term Care (LTC), Skilled Nursing Facilities (SNF), nursing homes, or similar settings.
Strong clinical skills in medication administration, wound care, and resident assessment.
IV therapy/IV certification preferred or willingness to obtain certification when required.
Experience caring for geriatric residents and individuals with chronic medical conditions, dementia, or developmental disabilities is highly desirable.
Ability to recognize changes in residents' conditions and respond appropriately.
Strong knowledge of infection-control practices and patient-safety standards.
Familiarity with electronic medical records and medication/treatment documentation.
Excellent communication and interpersonal skills.
Ability to work effectively as part of a multidisciplinary healthcare team.
Strong organizational skills and attention to detail.
Ability to work weekends and flexible shifts as required by the facility.

Pay: $1,500.00 - $1,800.00 per week
