Job Overview

We are seeking a compassionate and dedicated Certified Home Health Aide to join our dynamic healthcare team. In this vital role, you will provide personalized, high-quality care to patients in their homes, fostering independence and enhancing quality of life. Your energetic approach and attention to detail will ensure that each patient receives attentive support tailored to their unique needs. This position offers an opportunity to make a meaningful difference in the lives of individuals requiring assistance with daily activities, medical needs, and emotional support.

Duties

Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using patient transfer methods and equipment such as Hoyer lifts.
Monitor and record vital signs accurately, including blood pressure, pulse, respiration, and temperature, ensuring timely communication of any concerns.
Administer medications as prescribed in accordance with home healthcare regulations and document all medical records diligently using Electronic Health Record (EHR) systems.
Provide specialized care for patients with dementia, Alzheimer’s disease, or other cognitive impairments, including memory care and behavioral management strategies.
Support patients during end-of-life care with compassion and respect, maintaining patient confidentiality in compliance with HIPAA regulations.
Perform light housekeeping tasks such as cleaning, laundry, and meal preparation to promote a safe and comfortable environment.
Assist with patient transfers and mobility using proper techniques to ensure safety for both the patient and caregiver.

Skills

Proven experience working with geriatric patients, individuals with developmental disabilities, or those requiring hospice or palliative care.
Strong knowledge of medical terminology, medication administration protocols, and basic life support (BLS) including CPR.
Familiarity with EMR/EHR systems for accurate documentation of patient care activities and medical records.
Excellent patient observation skills to detect changes in condition or behavior promptly.
Ability to communicate effectively with clients, families, and healthcare teams while maintaining patient confidentiality under HIPAA guidelines.
Experience working within home health care agencies or long-term care facilities is highly desirable.
Physical stamina for heavy lifting, assisting with transfers, and providing hands-on caregiving in a variety of home environments.
