Full Job Description

Clarion Forest VNA is seeking a dedicated Registered Nurse (RN) to oversee a primary caseload of Home Health patients. This role involves providing skilled professional nursing care as prescribed by physicians and in compliance with Medicare Conditions of Participation, Pennsylvania licensure laws, and agency policies. The RN will play a vital role in patient care, from comprehensive assessments to implementing and evaluating personalized care plans.

Responsibilities:

Conduct thorough assessments and develop individualized patient care plans.
Provide nursing interventions, therapeutic procedures, and education to patients and their families.
Collaborate with physicians and team members to optimize patient outcomes.
Manage and document patient care effectively, ensuring compliance with all regulations.
Act as a mentor and educator for team members and participate in training initiatives.
Participate in performance improvement initiatives and case management activities.
Maintain a professional and empathetic approach to patient and family interactions.

Qualifications:

Graduate of an accredited nursing program; BSN preferred.
Active RN license in Pennsylvania.
At least one year of general nursing experience; Home Health, medical/surgical, or critical care experience preferred.
Valid driver’s license and reliable, insured transportation.
CPR certification.
Strong communication, organizational, and critical thinking skills.

Physical Requirements:

Ability to perform tasks such as walking, bending, and lifting up to 50 lbs.
Comfortable working in various home environments and settings.
Visual and auditory acuity sufficient for written and verbal communication.

Hours are primarily Monday-Friday 8:00AM-4:00PM plus 1 weekend/month, 2 Holidays/year and rotating evenings. Evening hours are scheduled visits between 4:00PM - 9:00PM as needed.

Salary Range: $54,000 to $70,000 Annually with a generous bonus system in place. Salary commensurate with education and experience.
