Full Job Description

Job Title: Care Connection Assistant
Pay: $24.75 per hour
Reports To: Community Health Program Manager
Status: Non-Exempt, On-Site, Full-Time

Job Summary: The Community-Based Care Coordination grant that funds this position will be up in December 2026. Asotin County Health District (ACHD) intends to secure funding for the position to continue after 2026, either through the same funding source or through its own means. The Care Connection Assistant (CCA) will work to eliminate or mitigate barriers to health and social services for community members by serving as an advocate and link to connect clients with resources and services. They will also assist clients in accessing these resources, providing services in and around the community, as well as in the office.

Qualifications:

High School Diploma or GED
Valid U.S. driver's license

Essential Functions:

Maintain a regular caseload of clients who qualify for this assistance
Compliance with program grants and contract
Work out in the community, to be readily available and accessible to clients
Serve as a client advocate and liaison to reduce barriers to care
Refer clients to appropriate resources based on client-identified health and social service goals
Assist clients with resource navigation such as completing applications or finding transportation
Establish trusting relationships with referred clients and community partners
Maintain ongoing assessment of client and family needs to meet expected outcomes
Attend regular staff meetings, trainings, and other meetings as required
Other duties as assigned

Excellent written and verbal communication skills, including the ability to modify language to meet clients' needs
Ability to work with people who may have different lifestyles and priorities than your own
Ability to work independently and collaboratively
Ability to learn and use computer programs
Ability to retain confidential information; ensure the security of sensitive data
Ability to act with integrity and professionalism
Excellent relationship-building skills
Excellent ability to set firm boundaries with clients while maintaining a high level of service

Working Environment/Physical Abilities:

Work with distractions and frequent interruptions
Share an office with one other CCA
Shared office space with other employees
Fast paced environment, with sitting or standing for long periods of time
Lift up to 30 pounds
Sufficient hearing and speech to communicate in-person and on the telephone with or without reasonable accommodation
Manual dexterity and visual acuity to operate a computer station and other machines with or without reasonable accommodation
