Full Job Description

Responsible to CovenantCare Practices’ Quality Department and the patients of the practice for supporting a variety of quality improvement activities in an effort to provide the best patient care, reduce healthcare costs and improve population health. This role partners with the Patient Care Coordinator focused on Transitions of Care/Transitional Care Management (TOC/TCM) to ensure comprehensive patient outreach coverage across the Quality Department's value-based and pay-for-quality programs.

Patient Care Coordinator Job Responsibilities

1. Conducts outbound Chronic Care Management (CCM) outreach and enrollment calls, working assigned eligible-patient lists to enroll new participants and complete monthly billable CCM touches.

2. Completes monthly CCM patient calls and clinical time documentation to support accurate billing under CPT codes 99490, 99439, 99487, and 99489.

3. Monitors and works the clinical documents inbox daily, reviewing incoming consult notes (e.g., diabetic eye exam, colonoscopy, and specialist referral notes) for evidence of quality gap closure.

4. Prepares and submits closed quality-gap CPT codes to the Billing department with appropriate supporting documentation for ACO, TennCare, and Medicare Advantage payer programs.

5. Conducts quality gap-closure outreach calls to patients (A1c, eye exams, colonoscopy, blood pressure, depression screening, and other measures) to schedule needed visits and services.

6. Sends and tracks CCM contact letters for unreachable patients and documents all outreach attempts in AthenaNet alert/case notes.

7. Supports patient education and outreach campaigns, including Phreesia messaging and social media (Facebook) promotion of CCM and preventive services.

8. Assists with ADT report review and the ED/Hospitalization follow-up campaign, contacting patients after discharge or ED visits to close care gaps and encourage follow-up care.

9. Prioritizes outreach to high-risk and high-ED-utilizer patients as identified by the Quality Manager.

10. Coordinates with front office and clinical staff to schedule patients identified through outreach for needed appointments.

11. Maintains accurate, up-to-date records of all outreach attempts, enrollments, and gap-closure activity in AthenaNet and any designated tracking tools.

12. Communicates regularly with the Quality Manager on outreach progress, barriers to patient contact, and billing submission status.

13. Maintains the strictest confidentiality regarding patient, employee, and physician information.

14. Attends required meetings and participates in Quality Department staff meetings as requested.

15. Performs other duties as requested by the Quality Manager or administrative staff.

Patient Care Coordinator Job Qualifications

§ Required: Associates Degree in health sciences, business, health information technology, nursing, or the equivalent in related work experience.

§ Desired: Bachelor’s Degree in health sciences, nursing, business, healthcare administration, or health information management.

§ Two or more years of experience in a medical office, call center, or patient outreach/engagement role preferred

§ Knowledge of Microsoft Word and Excel

§ Familiarity of National Committee for Quality Assurance (NCQA) structure and standards, Healthplan Employer Data and Information Set (HEDIS) requirements, and any accreditation standards.

§ Familiarity with AthenaNet preferred.

§ Comfortable with high-volume outbound phone outreach and repetitive call workflows.

§ Strong verbal communication skills and patient, empathetic phone manner.

§ Excellent organizational skills and attention to detail, particularly in documentation and data entry

§ Displays excellent communication and problem-solving skills.

§ Ability to work independently, manage a call/task queue, and meet monthly outreach and documentation targets.

Physical/Mental Demands:

The physical requirements described represent those that must be met by an employee to successfully perform the essential functions of the job. While performing the duties of this job, the employee is regularly required to sit and stand, use hands to manipulate objects or keyboard, reach with hands and arms, and to be able to speak and hear. The employee must be able to lift and/or move 10-25 pounds. Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception, and the ability to adjust focus. Noise level in the work environment is usually quiet to moderate. Works in a medical clinic with computers and printers, over 25 employees at a time, patients, and medical equipment. The employee must be able to concentrate on task at hand while multitasking on other clinic improvements.

Pay: $18.00 - $21.00 per hour
