Summary: The Therapist provides integrated behavioral health services within outpatient and primary care partnership settings. This role delivers assessments, brief interventions, individual and family therapy, crisis evaluations, and collaborative care coordination for children, youth, adults, and families. The Therapist implements a Behavioral Health Consultation (BHC) model while working closely with medical providers, school systems, and community partners to ensure trauma informed, strengths based, wraparound support.

Essential Duties & Responsibilities:

Clinical Service Delivery

Implement the Behavioral Health Consultation (BHC) model in primary care and outpatient settings.

Provide brief interventions, behavioral interventions, and crisis/risk assessments (suicidal/homicidal).

Conduct substance use screenings and interventions when clinically necessary.

Deliver individual, family, and group therapy as identified.

Provide anxiety, trauma, stress management, anger management, and health behavior change support.

Assessment & Treatment Planning

Complete diagnostic interviews and other assessments to determine eligibility and level of care.

Complete Strengths-Based Assessments and develop Individual Service Plans for children, youth, adults, and families.

Complete evidence-based measures and track therapeutic progress.

Create and update treatment plans within the EHR following agency policy.

Collaboration & Care Coordination

Collaborate with primary care providers, hospital teams, and community partners to ensure wraparound support.

Provide providers with updates regarding client needs and progress.

Offer mental health consultations within clinical offices, hospitals, and related facilities.

Work with school systems, social services, and other child serving agencies as needed.

Training & Community Support

Provide trauma informed practice training to staff within clinics, hospitals, and community sites.

Deliver or support community trainings such as Youth Mental Health First Aid.

Documentation & Compliance

Complete transition and discharge summaries.

Provide treatment summaries and other documentation as requested.

Ensure all treatment services meet CARF standards.

Perform additional duties as assigned.

Qualifications:

Master’s degree in counseling, therapy, or closely related clinical field.

License preferred; license-eligible required.

Minimum two (2) years of clinical experience.

Experience in child-serving systems and non-profit organizations preferred.

Preference for candidates who are parents/caregivers of children with SED.

Non-profit organization experience and work history within child serving systems preferred

Valid driver’s license and current auto insurance required.

Other Skills and Abilities:

Understanding of developmental, emotional, relational, and social needs of children.

Experience supporting children and youth with mental health and behavioral challenges.

Knowledge of trauma-informed care for children, adolescents, and adults (including grief and loss).

Experience creating strengths-based treatment plans and supporting family skill-building.

Ability to empathize with and empower families.

Sensitivity to culture, family structures, and local community systems.

Knowledge of educational systems and other child-serving resources.

Strong organizational, written, communication, customer service, and problem-solving skills.

Ability to work independently and collaboratively.

Core Competencies:

Completes all assessments required for program standards (at intake and discharge,) in collaboration with the youth, family and Family Support Specialist (FSS) to identify strengths and needs

Works as a team with the Family Support Specialist to ensure coordinated service delivery and face-to-face contact with family/youth according to program standards.

Attends court/CFTMs and communicates with Youth Service Officers regarding youth/families’ participation in services and progress in completing goals established to address issues contributing to court involvement/delinquent charges

Assist FSS in identifying community resources and helping family engage with resources that will sustain family’s progress beyond discharge from JJR program

Available by cell phone for crisis support during working hours, provide 24/7 crisis support information and safety plan

Documents progress, safety plan, treatment plan, etc. and enter into the EHR

Assist in completing transition plan/discharge summary

Ensure client information is secured consistent with HIPAA requirements

Performance Evaluation: Employees are evaluated on an annual basis. Performance Reviews are completed electronically by the employee’s direct supervisor then reviewed and approved by regional or program leadership. Employees are given the opportunity to review their performance evaluation with their supervisor and will be required to acknowledge receipt of the annual review electronically.
