Chronic Care Staffing is Hiring a Care Coordinator - CMA or LPN !

Location: Remote (MST or PST time zones ONLY)
Schedule: Monday–Friday, no weekends

Overview

Managing a chronic illness can be overwhelming for patients. At Chronic Care Staffing, we believe that consistent, proactive support makes all the difference. We are seeking a compassionate, detail-oriented Care Coordinator (CMA or LPN) to join our team in a fully remote capacity. In this role, you will act as a trusted clinical contact for patients, helping them understand their care plans, manage their medications, and stay actively engaged in their health journey.

What Success Looks Like

On a day-to-day basis, success means managing your outreach queue with empathy and efficiency. You will spend your day calling patients, conducting health risk assessments, and updating Electronic Health Records (EHR) with high accuracy. You will serve as the bridge between patients and their healthcare providers, identifying potential barriers to care and escalating urgent clinical concerns when necessary.

Over time, success in this role is defined by improved patient engagement and healthier patient outcomes. By building trust during your monthly touchpoints, you will help patients stick to their care plans, reduce unnecessary hospital visits, and feel genuinely supported in managing their health.

Key Responsibilities

Conduct structured monthly outreach calls to monitor patient progress and encourage continuous engagement.
Verify medication adherence, perform health risk assessments, and document clinical updates.
Support Chronic Care Management (CCM), Transitional Care Management (TCM), and Remote Patient Monitoring (RPM) initiatives.
Ensure all patient interactions and care plans are documented accurately within client EHR systems.
Facilitate clear, professional communication between patients, family members, clinical providers, and internal teams.
Recognize and resolve barriers to care, escalating patient clinical issues to the appropriate provider when needed.
Educate patients and families on established care plans, treatment goals, and community resources.
Maintain strict confidentiality, adhering to HIPAA regulations and CMS documentation standards at all times.

Position Requirements

Must-Have Qualifications

Residency within the Mountain (MST) or Pacific (PST) time zones.
Bilingual fluency in English and Spanish.
Active, unencumbered Certified Medical Assistant (CMA) or Licensed Practical Nurse (LPN).
Current Basic Life Support (BLS) certification.
Recent clinical experience, preferably in care coordination, chronic care management, or a busy outpatient clinic.
Strong digital literacy, including hands-on experience with EHR systems and Google Workspace.
A dedicated, quiet, HIPAA-compliant home workspace with a reliable high-speed internet connection.
Excellent verbal and written communication skills, with a natural ability to connect with diverse patient populations.

Nice-to-Have Qualifications

Prior experience working with CCM, TCM, or RPM programs.
Knowledge of community-based health resources and social determinants of health.
Successful experience working independently in a remote or virtual healthcare setting.
Familiarity with CMS guidelines, compliance standards, and quality reporting measures.
