Home Health Care Intake Coordinator

About Us

We are a fast-growing, patient-focused home health agency committed to delivering exceptional care with efficiency, accuracy, and compassion. Our team thrives on clear communication, strong coordination, and a commitment to doing things right the first time.

Position Overview

The Intake Coordinator is the first point of contact for new patients, referral sources, and insurance partners. This role ensures that every patient enters our system smoothly, accurately, and with all required authorizations in place. You will manage the intake workflow from referral to admission, working closely with clinical, administrative, and billing teams.

Key Responsibilities

Receive, review, and process all incoming referrals from hospitals, physicians, facilities, and community partners.
Verify patient eligibility and benefits, including confirming coverage for home health services.
Obtain prior authorizations from insurance providers, ensuring timely approval for all required services.
Enter and manage patient information in WellSky (Kinnser) EMR, ensuring accuracy, completeness, and compliance.
Communicate with patients and families to gather necessary information and explain the intake process.
Coordinate with clinical staff to schedule start-of-care visits and ensure smooth handoff.
Maintain organized documentation and follow up on outstanding authorizations or missing information.
Serve as a liaison between referral sources, insurance companies, and internal departments.

Qualifications

Experience in home health intake, medical office administration, or insurance authorization strongly preferred.
Proficiency with WellSky EMR (required or strongly preferred).
Strong understanding of insurance verification and authorization processes.· Excellent communication, organization, and multitasking skills.
Ability to work in a fast-paced environment with accuracy and professionalism.· Compassionate, patient-focused mindset with strong attention to detail.
