Full Job Description

Summary:

The Clinical works in cooperation with Ohio Care Access Administration. The Clinical will be responsible to supervise and assists administration in the development and implementation of policies and procedures relating to medical treatment, infection control, and individual care.

Major Job Duties:

Summary of the major duties and responsibilities.

Willing to travel to our office in Middleburg Heights, and occasionally travel to our three group homes in Richmond Heights and Bedford·
Implements changes in the healthcare records and/or treatment plans of individuals assigned as required.
Consults with physicians, pharmacists, and other medical personnel as needed.
Serves as a consultant to agency staff regarding the health and welfare of individuals, including on call.
Assists the Administration in the development and implementation of policies and procedures relating to medical treatment, infection control, and individual care.
Participates in ensuring compliance with regulatory requirements as they relate to health and safety.
Serves as a consultant to agency staff regarding the health and welfare of individuals, including on call.
Serves as liaison to schools, workshops, non-agency nurses, physicians, parents, etc. regarding health and safety issues.
Participates in CPR/First aid, Infection Control, Medication Administration and any other employee or individual training, as needed to maintain regulatory compliance.
Assists in the Quality Assurance process in the areas of consumer healthcare, medications, medical records and other healthcare records.
Participates in QA Audits and develops a Plan of Correction if needed.
Attends medical, dental, PT/ OT, psychiatric and other resident appointments as deemed necessary
Transports consumers to medical appointments as necessary and makes follow up notifications to the necessary individuals within 24 hours
Provides emergency medical care in a prompt manner.
Responds promptly and courteously to family concerns.
Provides on-call services for consultation of consumer and nurse staffing needs.
Works in conjunction with others to implement and maintain the Medication Administration Nursing Program and coordinates Med Pass Check Offs for caseload of homes.
Provides healthcare training to staff and consumers as required.
Submits regular reports regarding resident medical status as requested.
Manages directly the health and safety of a caseload of homes and makes routine visits to the homes
On Call medication passing as necessary
Routinely monitors physicians orders to ensure accuracy on MAR
Perform monthly cycle change over
Performs med-cart audits to ensure cart is orderly (i.e.- remove expired meds and discontinue meds)
Prepare referral forms for medical appointments needs
Track routine consumer appointment needs
Ensure all new physicians orders are implemented in a timely manner
Monitor status of consumers in hospital/rehab on a regular basis by visiting and ongoing communication with facility and report to team
Participants in discharge planning when consumers are released from hospitals or other facilities
Attend ISP meetings for new consumers and existing consumers as necessary
Performs other related duties as assigned

Minimum Qualifications and Requirements:

· Age 21 or older

Must have high school diploma or GED

· Meet DSP job and skill requirements, attributes and values.

· Ability to lift 50 pounds and safely lift, turn, position and transfer individuals with physical disabilities.

· Must have solid communication skills, both oral and written.

· Must have solid computer skills and understanding of MS Word, Excel and e-mail.

· Must have strong organizational, problem solving and time management skills.

· Must demonstrate integrity and ability to lead others.

· Must have a belief in and understanding of agency’s culture and mission and ability to promote and communicate the philosophy, mission and values to internal and external stakeholders.

Must have reliable transportation, a valid Ohio driver’s license with four points or less
