Full Job Description

SUMMARY:

The Clinical Floater Pharmacist position is a hybrid role that incorporates both clinical MTM services as well as in-store retail community pharmacy.

This position will be responsible for providing comprehensive Medication Therapy Management (MTM) services to improve patient outcomes, optimize medication utilization, and support quality care initiatives. This role will support clinical initiatives including comprehensive medication reviews (CMRs), targeted interventions (TIPs/TMRs), immunization opportunities (including supporting immunization clinics), disease state management, and patient education.
The Pharmacist is also responsible for floating at our retail pharmacies in the area. Responsibilities would include dispensing prescribed pharmaceuticals, providing information and counseling to customers while maintaining outstanding customer service and safeguarding company assets.

Job Details:

Full-time, 40 hours
Monday-Friday (may include occasional weekends)
Schedule will be split between our Clinical Contact Center and our retail store pharmacies.
2 days in clinical setting, 3 days in retail setting (Rotates every other week)
3 days in clinical setting, 2 days in retail setting
The Clinical Contact Center is located in Avon Lake, OH
This position requires floating at Discount Drug Mart retail pharmacies .
Locations may include: Avon Lake, Avon, Westlake, Elyria, Lorain, North Olmsted, etc.

Pharmacy is closed on major holidays: New Year's Day, Easter, Memorial Day, July 4th, Labor Day, Thanksgiving, & Christmas Day

PRIMARY/ESSENTIAL DUTIES AND RESPONSIBILITES

Practice the profession of pharmacy in a manner consistent with ORC Chapter 4729 in addition to all state and federal laws and regulations as amended and promulgated
Arm and unarm pharmacy and facility security upon entering and leaving
Perform CMRs and TIPs/TMRs for eligible patients with high accuracy and quality
Interpret, data entry, review, fill, check, dispense, and accept responsibility for prescriptions written by physicians for our patients.
Prepares or supervises the preparation of liquid medicines, ointments, powders, tablets and other medication along with checking prescriptions for correct dosage and for drug interactions and incompatibilities. Compounds prescribed medicines and packages and labels them.
Identify and resolve medication related problems including therapeutic duplications, adherence concerns, drug interactions, gaps in care, and high risk medication use
Contact patients, prescribers, and retail store locations for therapy optimization and improved medication outcomes
Document all MTM interventions accurately and efficiently within applicable MTM platforms and pharmacy systems
Monitor and improve patient adherence through medication synchronization (SYNC your MEDS) and patient counseling
Ensures availability of merchandise and services and maintains inventories.
Provides immunization services (counseling, selection, and immunizing). Identify opportunities for immunizations and attend immunization clinics
Support quality metrics and payor performance programs, including Medicare Star Ratings
Adhere to pharmacy regulations and HIPAA requirements following facility’s policies and procedures
Assist with the development and implementation of MTM workflows, best practices, and one on one trainings
Participate in meetings, training programs, and quality improvement initiatives
Track and report MTM productivity
Support pharmacy operations, Central Fill pharmacy production, and clinical initiatives as assigned
Other duties or projects as assigned leadership

QUALIFICATIONS

Pharmacy degree from an accredited college of pharmacy with Pharm D Degree or BS Degree.
Possession of a current unrestricted and valid Pharmacist license issued by Ohio State Board of Pharmacy, and a minimum of 2 years of professional experience as a Pharmacist
Previous MTM or clinical pharmacy experience preferred
Immunization certification required
Strong clinical knowledge, problem solving, and organizational skills
Ability to manage multiple priorities in a fast paced environment
Experience with MTM platforms, PioneerRx, and Microsoft Office

ESSENTIAL POSITION REQUIREMENTS AND SKILLS

Ability to read and follow directions for all required tasks
Perform repetitive tasks with accuracy as assigned
Ability to regularly lift and/or move up to 10 pounds, frequently lift and/or move up to 25 pounds, and occasionally lift and/or move up to 50 pounds
Ability to work varied hours/days as business dictates
Ability to analyze patient medication profiles
Ability to operate and use all equipment necessary to fill prescriptions within the Central Fill Pharmacy
Ability to multi-task, prioritize, and be flexible with workload
Effectively communicate with patients, prescribers, other employees, and management to ensure tasks are completed as assigned
Job duties will require the employee to sit, type on a keyboard, and be on the phone for extended periods of time
Ability to maintain confidentiality by law and corporate policy
Occasional travel for immunization clinics, trainings, and meetings
Adapt to a rapidly changing environment
Ability to work independently and in small teams
