Position Purpose:
The primary purpose of the position is to serve as the lead team member for Marjorie P. Lee’s (MPL) residential assisted living memory support household. This position is responsible for leading the assigned household team members, projects, and processes that are in place to promote resident quality of life and quality of care on MPL’s residential assisted living memory support household within ERS’s policies, procedures, and standards.
Minimum Qualifications:
- Possesses, at a minimum, a high school diploma/GED. Associates degree preferred.
- Current valid certification as a State Tested Nursing Assistant in Ohio.
- Prior Leadership experience preferred.
- Life enrichment / activity experience preferred.
- Ability to secure Activity Director Certification within 6 months of working within the position
- Minimum of 3 years’ experience working with older adult populations and dementia.
- Strong competency with Microsoft Office Suite products (word, outlook, excel, etc.)
- Moves equipment and supplies weighing up to 50 lbs. on a regular basis and up to 100 lbs. on an occasional basis
- Has the ability to relate to and work effectively with the ill, disabled, elderly, emotionally upset and, at times, hostile people within the household and to remain calm in emotionally charged situations.
- Is able to read, write and communicate effectively in the English language.
- Is able to see and hear, with or without reasonable accommodation, so that the essential functions of this position may be fully met.
- Is able to sit, stoop, bend and move continuously throughout the day.
- Is able and willing to work independently with flexibility and integrity, as well as effectively and harmoniously with team members, residents, family members, volunteers, government agencies/personnel.
- Possesses leadership ability and is willing to communicate with residents based at whatever behavior or acuity level they are currently functioning.
- Has knowledge and competencies for age specific interventions.
- Must be fully up-to-date with required vaccinations.
As the Memory Support Household Coordinator you can expect to:
- Responsible for management and leadership of assigned team members, including but not limited to interviewing, orienting, mentoring, evaluating, managing, and counseling.
- In partnership with scheduler, helps lead scheduling and payroll processes for household, including assuring appropriate team member coverage at all times.
- Provides leadership and vision to the household by serving as a role model for core competencies and person-centered care.
- Operates with a servant leadership mentality, taking an active and engaged role in the household with both residents and team members. Partners with team members on tasks including but not limited to assisting with residents’ personal care needs.
- In partnership with the Director of Nursing, Executive Director of Hospitality Services, and Administrator, communicate and hold team members accountable to following clinical, dining, programming, and infection control policies and procedures, etc.
- Conducts routine one on ones with assigned team members.
- Communicates effectively with team members, residents, families, and care partners by reading and responding promptly to messages on a daily basis.
- Serves as the SAIDO Chief Lead for MPL and helps guide the community’s SAIDO strategy. Leads SAIDO training initiatives for MPL.
- In partnership with nursing and social services, ensures that necessary appointments are made, and that travel and other support is arranged to enable residents to attend such appointments. Facilitates periodic in-house appointments with podiatry, dental, vision and audiology consultants.
- Ensures household is kept adequately stocked with medical supplies and office supplies and works with other departments to restock as needed.
- Submits work order requests as needed throughout household in common areas, resident rooms, kitchens, equipment, etc.
- Maintains a safe, comfortable, inviting, and functional environment by routinely assessing the household, including both common areas and resident apartments, for potential hazards, safety concerns, and cleanliness. Follows up to assure identified concerns are addressed timely.
- Ensures safe work habits, and ensures ERS’ policies and procedures for sanitation, infection control, disaster preparedness, and emergency care are followed.
- Partners with Move-In Coordinator as point person on the assisted living memory support household during resident move-ins and helps orient family and resident to household. Creates plan to integrate resident into household and provides family with proactive routine updates.
- Holds pre-move in realistic expectations meetings with families.
- Leads and provides timely documentation of monthly resident council for Assisted Living Memory Support household. Follows up timely with appropriate team members on any recommendations made during these meetings.
- Assists Community Relations in Residential Memory Support marketing initiatives and events. · Completes and submits reports and requests for information as directed.
- Ensures household budgets remain on target.
- Plans, develops, implements, and evaluates innovative, person-centered dementia programming designed for both individuals and groups to promote engagement, purpose, connection, and quality of life for residents living with cognitive impairment.
- Assumes primary responsibility for creating and maintaining a consistently active, engaging, and lively household environment by developing and facilitating person-centered individualized and group engagement opportunities that reflect residents' life stories, interests, preferences, strengths, and abilities, fostering meaningful experiences and purposeful participation throughout the day. Leads assigned team members in assuring compliance with programming and engagement expectations.
- Encourages residents to attend programming and assists in transporting residents to life enriching opportunities of their choice both within and outside the community as appropriate.
- Partners with community leadership and ERS’s Center for Memory Support and Inclusion to develop, coordinate, and implement dementia-focused initiatives (e.g., Dementia Live), quality improvement projects, special programs, and educational opportunities that enhance the overall resident experience and strengthen team member knowledge, skills, and confidence in providing person-centered dementia care.
- Maintains a monthly Assisted Living Memory Support programming calendar by established deadlines and ensures residents, team members, and families are informed of scheduled activities and engagement opportunities. Proactively shares updates and information on day-to-day household life with residents and families while maintaining compliance with HIPAA.
- Partners with team members across community to implement and offer life enrichment programs.
- Schedules, facilitates and participates in care conferences in partnership with social services team for household residents at required intervals and as needed.
- Maintains and monitors resident schedules (e.g., shower), resident preference binders, team member shift reports, and other documentation. Records all resident-related participation, assessments, and care plan documentation, and completes other paperwork and charting in a timely and accurate manner and in compliance with State of Ohio and ERS guidelines. Ensures assigned team members do the same with all required documentation components.
- In partnership with nursing leadership, completes ongoing assessments of resident’s spiritual, social, physical, and emotional well-being as well as their participation in life enrichment activities. Utilizes the information to develop and implement a plan of care as needed.
