Our services include primary and pediatric care, dental care, behavioral and SUD care, supportive housing, and community re-entry services, to name a few. Central City Health is committed to enabling every patient in our community to thrive by building a collaborative multidisciplinary healthcare team. The Peer Support Specialist provides peer support services to clients experiencing mental illness, chronic health condition(s) and/or homelessness.

General Summary:
Central City Health (CCH) is committed to enabling every patient in our community to thrive by building a collaborative multidisciplinary healthcare team. The Vaccine Coordinator is responsible for overseeing the vaccine program. The Vaccine Coordinator is responsible for maintaining compliance with vaccine management. Vaccine management includes vaccine ordering, inventory and reconciliation, storage and handling, temperature monitoring, transfer of vaccines, administering and managing an emergency plan.

Responsibilities

Manages vaccine inventory by identifying vaccines upon receipt, account for total inventory supply, rotate stock and records temperatures.
Performs an inventory of vaccines on hand and determine doses administered since the date of the last reconciliation; resolves all vaccine discrepancies before informing Leadership.
Orders vaccines based on population, need and usage. Monitors and reports expired and wasted vaccines.
Maintains all documentation regarding vaccine storage and handling (including maintenance work/repairs); monitors the temperatures and contacts Leadership if temperature rise above or below acceptable temperature ranges.
Reconciles vaccinations on a weekly, monthly and/or yearly basis or when requested by Leadership.
Oversees proper vaccine transport when transport is necessary for off-site immunization clinics, in emergency situations, or to transfer vaccine to another provider office as approved.
Partners with Leadership to ensure that all staff receive adequate education and training.
Partners with Leadership to maintain a Vaccine Management Plan and an Emergency Plan for Vaccine Relocation.
Participates in various primary care activities and assists with special projects as needed.
Assists in preparation of patient for examination obtains height, weight and vital signs and records in the medical records.
Records patient vaccine and immunization information in MCIR and obtain MCIR results/reports as needed.
Assists physicians/nurses with diagnostic procedures, treatments, and examinations.
Assists in the medical office as needed.
Assists with preparing exam rooms with equipment and supplies as needed for examination procedures, cleans, and restocks examining rooms as necessary. Maintains work area in a neat and orderly manner.
Performs other duties as assigned.

EDUCATION AND EXPERIENCE

High school diploma or GED required.
Graduate of a Medical Assistant or Phlebotomist program required.
Medical Assistant or Phlebotomist certification preferred.
Experience with a vaccination program highly preferred.
Cardiopulmonary and Basic Life Saving (BLS) certification required;
Data entry experience preferred.
Proficiency with Microsoft Office, and patient electronic medical record systems.
Must possess knowledge of the healthcare field including medical terminology.
Familiarity with medical laboratory and Michigan Care Improvement Registry (MCIR).
Must be able to work cooperatively with all involved in delivery of service to patients.
Must maintain confidentiality relating to treatment in accordance with HIPAA.

Pay: Up to $25.00 per hour
