Full Job Description

Clinical Manager (RN) – Home Health

Location: Melbourne, FL
Job Type: Full-Time
Schedule: Monday–Friday

About the Opportunity

A well-established and growing Medicare-certified home health agency is seeking an experienced Clinical Manager (RN) to oversee clinical operations, support field staff, and ensure the delivery of exceptional patient care. This leadership role is ideal for a motivated RN with strong knowledge of Medicare home health regulations and a passion for developing high-performing clinical teams.

Responsibilities

Provide clinical oversight and supervision of field clinicians, including RNs, LPNs, Therapists, Medical Social Workers, and Home Health Aides.
Review and approve OASIS assessments, plans of care, physician orders, and clinical documentation.
Ensure compliance with Medicare Conditions of Participation, state regulations, and agency policies.
Monitor patient outcomes and quality measures to drive clinical excellence.
Collaborate with physicians, referral sources, and interdisciplinary team members to coordinate patient care.
Mentor, educate, and support clinicians through orientation, training, and ongoing competency development.
Participate in Quality Assurance and Performance Improvement (QAPI) initiatives and maintain survey readiness.
Address clinical escalations, scheduling concerns, and patient care issues.
Participate in an on-call rotation as required.
Perform skilled nursing visits and provide direct patient care as needed to support staffing, clinical operations, and continuity of care.

Qualifications

Active Florida Registered Nurse (RN) license required.
Minimum of two (2) years of Medicare Home Health experience.
Previous Clinical Manager, Director of Nursing, or supervisory experience preferred.
Strong knowledge of OASIS, Medicare regulations, and Conditions of Participation.
Experience with electronic medical records (WellSky/Kinnser).
Excellent leadership, communication, organizational, and problem-solving skills.
Ability to manage multiple priorities in a fast-paced environment.
