Job Title: Support Planner / Case Manager

Mind Kind LLC is a community-based organization providing person-centered Support Planning and Case Management services to adults and individuals with disabilities enrolled in Maryland Medicaid Long Term Services and Supports (LTSS) programs. Position Summary:
The Support Planner/Case Manager is responsible for delivering person-centered, conflict-free case management and support planning services to individuals enrolled in Maryland Medicaid LTSS programs, including CPAS, CFC, HCBOW, and other OLTSS programs. Support Planners work directly with participants, families, providers, and community partners to ensure services align with participant goals, needs, and preferences while maintaining full regulatory compliance.
Key Responsibilities:
Participant Engagement & Assessment
- Conduct comprehensive, person-centered assessments and ongoing reassessments
- Develop, implement, and monitor individualized Plans of Service (POS)
- Engage participants in goal-setting that promotes independence, choice, and self-direction
Case Management & Coordination
- Coordinate Medicaid-covered services, natural supports, and community resources
- Monitor service delivery to ensure quality, appropriateness, and medical necessity
- Advocate for participants’ rights, preferences, and access to services
Documentation & Compliance
- Maintain accurate, timely, and compliant documentation in LTSSMaryland and agency systems
- Complete required contact notes, assessments, service plans, and reviews within mandated timelines
- Adhere to MDH OLTSS regulations, COMAR requirements, and agency policies
Collaboration & Quality Assurance
- Participate in supervision, team meetings, case reviews, and quality improvement activities
- Collaborate with interdisciplinary teams, providers, and supervisors
- Identify and report risks, incidents, barriers to services, and changes in participant status
Ethics & Professional Standards
- Maintain strict conflict-free case management practices
- Uphold confidentiality, HIPAA standards, and ethical guidelines
- Demonstrate cultural humility and respect for diverse populations
Qualifications:
Required
- Bachelor’s degree in Social Work, Human Services, Psychology, or a related field ( 2 years of exprerience may substitute for education)
- At least 1 year of case management or care coordination experience
- Ability to meet Maryland OLTSS Support Planner/Case Manager requirements
- Reliable transportation and ability to travel within assigned service area
- Strong documentation, organization, and time management skills
Preferred
- Experience working with individuals with disabilities, older adults, or Medicaid LTSS programs
- Knowledge of CPAS, CFC, HCBOW, or other Maryland waiver programs
- Experience using LTSSMaryland or similar electronic systems
- Social Work or human services licensure (LGSW, LMSW, etc.)
