Position Title
Home Manager
Affinity Care Group, LLC appreciates your interest in our company. We require our staff to have a valid driver's license, current vehicle insurance in your name and your vehicle to work with our persons-supported.

Position Summary
Develop the home staffing schedule and ensure the staff shifts are always covered within optimal hours with supervisory assistance.
Work direct care shifts, as required. The Home Manager is required to step into ratio in event of no shift coverage.
Advocate on behalf of person-supported, where appropriate.
Communicate with the Nurses and/or RSM’s regarding any health or behavioral issues.
Review Medication Administration Records for completion and accuracy, and follow-up on corrections needed.
Oversee the following:
House staff to ensure that they are completing job duties efficiently.
Training of direct care staff, to ensure direct care staff completes all paperwork for training programs in the home.
Completion and/or follow-up on the completion of documentation needed, including but not limited to, Incident/Accident/Injury reports, Behavior Tracking Data Sheets.
Direct care staff to ensure person-supported specific goals and objectives are followed and addressed.
Direct care staff to ensure good health, hygiene, and grooming of person-supported, and that person-supported maintain adequate and appropriate personal belongings.
Direct care staff to ensure housekeeping is done and cleaning checklists are filled out.
Planning weekly/monthly activities for the consumers and following up with weekend staff about the activities participated in.
Create weekly schedules
Coordination and preparation of all person-supported transitions, including person-supported placements, visits, and movements.
Purchase and/or oversee the purchase of necessary household supplies, food, and consumer purchases.
Maintain all receipts including house purchase card, gas card, and consumer trust fund receipts.
Responsibilities:
Support and coach individuals with developmental disabilities in a supportive and therapeutic environment, which maintains health, safety, and well-being while developing growth and independence.
Apply individual programming and behavior support plans as written in the Individualized Support Plan, Behavioral Support Plan, and any other therapies received by person-supported such as Physical Therapy, Occupational Therapy, Registered Dietician Service, Speech, or Counseling.
Engage supported individuals in community job development skills as well as recreational opportunities.
Certify all opportunities provided each supported individual a greater sense of self, promoting independence, enhancing self-esteem and an overall person-centered perspective ensuring a quality of life.
Minimum Requirements
High School Diploma or GED required; Associates Degree or Medical Health Care Certificate preferred.
All Candidates MUST HAVE a valid driver’s license; vehicle insurance in your name, with a good driving record and your own transportation.
National Background Check Clearance and DIDD Abuse and Neglect Clearance
CPR and First Aid certification
Medication Administration certification
Must be willing and able to submit to random drug testing throughout employment with good results.
Crisis Prevention and Intervention Training
Must Have Home Management Experience!!!; however, 6 months working with DDA is preferred. We offer a complete Training Program.
Physical Requirements: Ability to lift at least 50 lbs., Moving, lifting, or transferring of person supported may involve lifting of or up to 100 lbs.
