About the Role

As the Patient / Client Enrollment Coordinator, you will serve as the first point of connection for prospective clients entering our practice. From the initial discovery call through enrollment, you will guide individuals through a clear, supportive journey that helps them understand our model, feel confident in next steps, and decide whether Upstream is the right fit for their health goals.

This role blends relationship-building, structured communication, organization, and follow-through. You will lead discovery calls, nurture prospective clients, coordinate evaluations, welcome clients into the clinic, and support onboarding — all while maintaining Upstream’s calm, grounded, and purpose-driven culture.

As a full-time team member, this role also supports the day-to-day flow of the clinic in collaboration with our Wellness Coordinator team to ensure the clinic runs smoothly and professionally.

Admissions and client connection are the primary focus of this role, with clinic support serving as a collaborative extension of the team.

This is a highly relational, trust-centered role and a critical one: for many clients, you will be their first impression of our practice.

What Success Looks Like

Success in this role is rooted in trust, clarity, and consistency — both in how prospective clients feel and in how the admissions process flows.

Admissions & Client Connection

Lead 15-minute discovery calls with clarity, warmth, and confidence
Build rapport quickly and help prospective clients feel seen, understood, and supported
Guide prospective clients through a consultative process that helps them move from curiosity to confident commitment
Clearly communicate Upstream’s functional medicine model and next steps
Convert 75% or more of discovery calls into Practice Member Evaluations (PMEs)
Enroll 95% of clients who are clinically cleared and ready to move forward after their evaluation

Nurture & Follow-Through

Serve as the primary point of contact between the discovery call and enrollment
Maintain proactive communication to ensure clarity, readiness, and momentum
Provide timely, thoughtful follow-through for every prospective client interaction
Ensure all pre-appointment paperwork is completed accurately and on time

In-Clinic Experience

Welcome clients warmly when they arrive for their in-person evaluation
Provide a brief office tour to create comfort, familiarity, and ease

Organization & Insight

Maintain accurate CRM documentation and track key admissions metrics
Monitor discovery call volume, conversion rates, and long-term enrollments
Help maintain accurate and full PME scheduling availability
Share trends, insights, and patterns with the Medical Director and COO

Qualifications

Experience in relationship-based client conversion
Exceptional verbal and written communication skills
A warm, relatable communication style that builds trust quickly
Strong organization, attention to detail, and follow-through
Comfort using digital tools and learning CRM systems
Ability to follow structured call flows and proven processes consistently
Grounded emotional presence, humility, ownership, and service mindset
Availability for a full-time, in-person schedule at our Colorado Springs clinic

Nice to Have:

Experience in functional medicine, wellness, or healthcare settings
Skills in motivational interviewing or consultative sales
Familiarity with tools such as Google Workspace, Monday.com, ActiveCampaign, or Cerbo
Personal interest in functional medicine or lifestyle health
