JOB TITLE: Lead Service Coordinator, ILOS-CS
REPORTS TO: Program Manager, ILOS-Community Supports
HOURS: 8am-4:30pm
CLASSIFICATION: Exempt
SUMMARY: The Lead Service Coordinator - will report to the Program Manager of Inland Empire ECM/ILOS Programs. The Lead Service Coordinator will carry an abbreviated caseload and provide day-to-day program coordination for the IEHP team with the support and full knowledge of the Program Manager. The ILOS Lead Service Coordinator position will take a lead role assigning and tracking program referrals to team members and provide status updates re: outreach and engagement, housing application assistance, intensive case management, linkage to care providers, and housing supportive services for members enrolled in IEHP, Molina, and Health Net.

Leadership, Administration, Oversight

Coordinate Line staff schedules for all members and ILOS activities such as home visits, move-ins, pending intakes, documentation, joint retention visits, etc.

Lead/Assist Once referred members are assigned, assist with outreach engagement, enrollment, intake, and assessment. • Assist with SCI onboarding and training.

Work collaboratively with other members of the ILOS team as needed to provide support and effectively perform their assigned job duties and contribute to the achievement of the program's goals and objectives of housing and income stability.

Attend staff meetings, supervision and training as required.

Attend community meetings as requested and communicate with non-Step Up providers as appropriate and professionally representing Step Up.

Other duties as assigned and required for program compliance and success.
Direct Service and Support

For assigned member cases: Provide field based intensive case management services that promotes wellness and recovery for members that are transitioning from homelessness to permanent supportive housing. Facilitate and link members to appropriate community resources.

Meet with members during regular home visits to review member's progress towards goals, and reassess and revise service plans using recovery oriented service models including Housing First Harm reduction to ensure stable housing and support independent living skills.

Link to educational and employment goals.

Liaison between member and housing for compliance with lease, eviction prevention, good neighbor behaviors, behavioral contracts, etc.
Documentation and Data Collection

File Compliance: Assist with file compliance and audit preparation in coordination with Director/Assistant Program Manager. Ensure as a Team responsibility all charts are in compliance at all times and audit ready in both paper and electronic versions. Report all non-compliance to Director for corrective actions.

Data Collection, HMIS and Reporting: Ensure accuracy of all Welligent data, provide reports to Executive management as requested. Alert to inaccuracies and request assistance for corrections.

Track and provide reports as requested on housing placements for all housed individuals.
SKILLS: Leadership, motivation and management desires or experience; Strong assessment, interpersonal and intervention skills. Accuracy in charting required documentation. Ability to train or guide staff to achieve confidence and ability to work independently in the field. Strong organizational skills and ability to delegate tasks to staff and monitor outcomes. Experience working with Homeless populations and field-based services. Must have the ability to effectively communicate both verbally and in writing. The ability to work as a part of a team is crucial. Must be organized and detail oriented. Ability to present in a professional manner the ILOS Program to community partners and advocate for SU members' success in ending homelessness. Ability to work with disabled populations in a recovery-oriented context, embracing housing first, harm reduction and motivational interviewing techniques to ensure successful housing placement and end to homelessness cycle. Demonstrate Step Up's core values in day-to-day work.
QUALIFICATIONS: Master's or Bachelor preferred. Experience working in case management with at I east four years' experience in a leadership role can replace the preferred education requirement.
Strong computer skills with knowledge of Microsoft Office and intermediate MS Excel skills are required. Position requires use of own or company vehicle. Must have a clean driving record and current California's driver's license and insurance. Agency will reimburse for mileage.
SUPPLEMENTAL INFORMATION: Work with people experiencing homelessness whether on
the streets, in shelters or other places of habitation or services, may present inherent challenges and difficulties such as: exposure to bed bugs or other infestations, unpleasant smells or odors, individuals who may have poor hygiene or unclean homes due to homelessness, mental health symptoms, or poverty. Additionally, in working with our members, employees may encounter instances of profanity, sexually explicit or derogatory language, or verbal or physical expressions of anger and trauma. Very rarely do these situations ever escalate to physical
contact, and negative or derogatory communication patterns can often be negotiated successfully. All direct care staff are provided adequate training to develop skills to deescalate crisis situations that may arise and Step Up has established procedural safeguards for all employees to mitigate (but not eliminate) these inherent risks of employment.
PHYSICAL REQUIREMENTS: While performing the duties of this job, the employee is regularly required to sit; use hands and fingers; handle or feel; reach with hands and arms; talk; and hear. The employee is frequently required to walk, balance, stoop, kneel, and/or crouch. The employee must occasionally lift and/or move up to 15 pounds. Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception, and ability to adjust focus. Keyboard data entry required.
