Full Job Description

Maintain an accurate record of available beds. This includes daily and monthly census reports as directed.

Admit, transfer and discharge residents in accordance with established policies and procedures.

Maintain a current listing of all residents, their level of care and assigned room numbers. Provide copies to such listing to nursing, service, administrative personnel as necessary.

Review all referrals promptly for possible admission to the facility. This will include reviewing all available medical information, interviews with medical personnel/family members. Making home/hospital visits as appropriate. Assure that Director of admissions/DON/or Administrator approves any referral with known behaviors/exit seeking/ and/or high cost medications, prior to making a bed offer. Assure LMCEC patients that are in the hospital and assigned to you, have notes reviewed prior to readmitting to assure we are able to meet the needs of the resident prior to the patients return/transport.

As part of the admission process: assign room number; notify nursing staff of anticipated admissions; collect, assemble and check admission papers; obtain the resident’s/guardian’s signature on all required permits, releases, authorizations, etc. Verify insurance coverage on all admissions and obtaining pre-certification on any that require it. Explain to resident and/or guardian, room rates, transfers, billing procedures, visiting hours, restrictions, as appropriate.

Assemble medical charts and review admission papers for accuracy and forward to appropriate departments/units.

Develop and maintain a good working relationship with inter-departmental personnel, as well as hospital personnel and other members of the community.

Keep abreast of current Medicare/Medicaid regulations governing admission/discharge requirements for health care facilities.

Maintain resident waiting list in accordance with established policies and procedures.

Will become familiar with medical conditions and services needed for assessing patients for new admissions. Be able to review information and offer beds to appropriate referrals.

Conduct tours of facility for potential admissions and interested members of the community. Be able to provide all necessary information related to areas of the facility, i.e., care provided, staffing, cost, special services unique to certain areas.

Will be available for community education in areas related to long-term care, Alzheimer’s disease and related subjects. Keep abreast of current information related to Alzheimer’s disease and treatment, and information related to long-term care services.

Will make visits to potential community referral sources. Be able to discuss all aspects of services available in our facility. This includes, but not limited to, Carroll Campbell Place, Rehabilitation Therapy, and Long-term Care Services.

Will focus on referral sources for Carroll Campbell Place, maintaining census and developing waiting list for future vacancies.

Assume the administrative authority, responsibility, and accountability of performing assigned duties of the Admissions Coordinator.

Other related duties and responsibilities that may become necessary to assure that the needs of the facility are met, to include oversight of the grievance program and the Quality Assurance associated with reporting oversight of the grievance program.

Educational Requirements

BS degree in Human Services or related field. And or 1 + years’ experience in admission/marketing/social work – preferably long-term care.

Experience

1 + years experience in admissions/marketing/social work. Preferably long-term care. Ability to work under pressure and meet deadlines. Excellent communication skills, attention to detail, and computer proficiency required.
