JOB DESCRIPTION: Full Time – Aftercare and Youth Services Coordinator
SUPERVISOR: Director of Programs and Services
HOURS: 40+ hours per week
CLASSIFICATION: Exempt, At-Will Position
The Aftercare and Youth Services Coordinator (AYSC) manages the comprehensive Aftercare Case Management for youth ages 15-24 that are exiting Palmetto Place’s Residential Programs and transitioning into stable, independent living. The AYSC will coordinate with Residential Case Coordinators to create an individualized transition plan and support each youth in successfully transitioning out of Residential Programming through independent living or family/fictive kin reunification. After their exit from the program, the ACC will engage in 12-months of wrap-around case management for each Aftercare youth, providing resource navigation, goal setting, community engagement, and independent living skill building, reducing the youth’s risks of returning to homelessness. This will require travel throughout the Richland and Lexington county to meet with youth in their community.
This role also works with community outreach, youth engagement, and program coordination. The AYSC plays a key role in implementing grant-aligned initiatives, including the Youth Engagement Advocates (YEA!) program.
Primary Responsibilities:
Aftercare Services (35%)
- Coordinate with Residential Program staff to plan and facilitate successful transition plans for youth exiting Residential Programs
- Provide 12-months of wrap-around Case Management for all youth transitioning out of Residential Programs
- Collaborate with the Department of Social Services to assist eligible youth in utilizing their Chaffee, ETV, and Independent Living benefits
- Ensuring fidelity to grant standards in Aftercare for the Basic Center Program, as established by the Family and Youth Services Bureau
- Co-facilitate monthly events for Community Youth to promote independent living, skill building, and foster community relationships
- Participate in a weekly Case Management meeting to review pending referrals, discuss Client concerns, and assess Client progress in a collaborative format
- Create content for the Youth Instagram, sharing resources, community events, and providing an engaging platform for community youth to connect
Safe Futures Outreach and Engagement (20%)
- Conduct outreach at designated community sites 2–3 days per week
- Engage youth in services and provide resource navigation
- Build and maintain partnerships with community organizations
- Track participation and support data collection and reporting requirements
Youth Engagement Advocates (YEA!) (25%)
- Oversee management of the YEA! Program, ensuring fidelity to grant standards as established by the Department of Social Services
- Facilitate monthly In-Person meetings for YEA! Members, utilizing the financial and meeting standards established by DSS
- Facilitate engagement, on-boarding, and sustainment efforts for YEA! participants, maintaining communication and reducing attendance barriers
- Submit completed and accurate paperwork for Monthly Reports, Youth Stipends, Activity Expenses and Travel Reimbursements
- Support DSS Training and Skill development activities at a State and Regional Level
Safe Place (10%)
- Develop, implement, and evaluate annual Safe Place goals and plans
- Assist Community Outreach Coordinator in recruiting businesses and public locations to serve as Safe Place sites
- Maintain community relations with key school personnel, business contacts, and community organizations
- Conduct school and other presentations to youth for the purpose of explaining the services offered by the agency
- Serve as the on-call liaison Monday-Friday
- Maintain Safe Place sites.
- Types of Sites Local networks of Safe Place sites should consist of both for-profit and not-for-profit locations. Businesses: fast food chains, convenience stores, grocery stores, malls, skating rinks, movie theaters, etc. Community services: transit systems, fire stations, libraries, community centers, YMCAs, Boys and Girls clubs, schools, healthcare clinics, youth service organizations, etc.
Residential Youth Support (5%)
- Provide backup coverage during staffing shortages, as neede
- Assist with supervision, daily routines, and crisis responses, as needed
Administrative and Reporting Support (5%)
- Maintain accurate and organized records across all programs inputting data within digital platforms such as Apricot
- Support data collection, reporting, and outcome tracking
- Utilize analysis of Client Progress, Goals, and Outcomes to provide informed decisions on program efficacy and organizational strategic planning
Additional Duties
- Attend staff meetings, retreats and professional training sessions and conferences
- Participate in Community Outreach to build partnerships that support the mission of the organization
- Completion of a minimum of 16 hours of training per year
- Perform other duties as assigned to meet organizational needs
Schedule:
This is a full-time role requiring a minimum of 40 hours per week, primarily during business hours (9 AM – 5 PM). Flexibility is essential to meet organizational and Client needs. Time is split between the two Midlands campuses, as well as across Richland and Lexington County to meet with clients in the community. This is not a remote position.
Qualifications:
- Bachelor’s degree in human services field or related field, plus 2 years of experience working with the population we serve. The Human Services field may include, but is not limited to, the following: psychology, social work, early childhood education, child development, or a related field, such as criminal justice, rehabilitative counseling, or elementary or secondary education.
- Strong Organizational and communication skills, including document management, time management, and compliance with DSS licensing and grant standards
- Communicate effectively and present professionally, advocate for the clients, have proficient writing skills, and navigate resources while forming partnerships within the community
- Proficiency in Microsoft Office, including Excel, Word and Outlook
- Excellent public speaking skills and ability to engage and build community partnerships
- Relationship Management skills- building and maintaining rapport with clients, peers, and community partners
- Ability to work autonomously and collaboratively in a team setting while working with various personalities to ensure the best intentions are held for the residents, staff, and organization
- Dedication to the mission of Palmetto Place and a passion for supporting youth (ages 15-24) while serving as a role model
- Ability to maintain flexible schedule to meet the needs of the organization
Pre-Employment Requirements:
- Clean and valid driver’s license and driving report
- Satisfactory reference checks
- Must be able to pass standard background checks including SLED, Fingerprint, DSS Central Registry, and both SC and National Sex Offender, drug screen, and employment physical
- If lived outside of South Carolina in the past 5 years, must also pass DSS Central Registry check from the state previously lived
- CPR and First Aid certification
- Must be 21 years of age or older
