Full Job Description

Job Summary:

The Registered Nurse (RN) in Home Care provides skilled nursing services to patients in their homes. The RN is responsible for assessing patient needs, developing and implementing care plans, providing direct nursing care, and educating patients and their families. The RN works collaboratively with physicians, therapists, social workers, and other members of the healthcare team to ensure quality and continuity of care.

Key Responsibilities:

- Conduct comprehensive assessments of patients’ health status and home environment.

- Develop and implement individualized care plans based on physician orders and patient needs.

- Provide direct nursing care, including wound care, medication administration, IV therapy, and disease management.

- Monitor and document patient progress, reporting changes to physicians and care team as necessary.

- Educate patients and their families on disease processes, self-care techniques, medication management, and prevention strategies.

- Coordinate care with multidisciplinary team members, including physical therapists, occupational therapists, and social workers.

- Maintain accurate and timely clinical documentation in compliance with agency policies and regulatory requirements.

- Participate in case conferences and care planning meetings.

- Adhere to infection control and safety procedures.

- Provide emotional support and advocacy for patients and families.

Qualifications:

- Current and valid Registered Nurse (RN) license in the state of practice.

- Associate’s or Bachelor’s degree in Nursing from an accredited program.

- Minimum of 1 year of clinical nursing experience (home care experience preferred).

- Current CPR/BLS certification.

- Valid driver’s license and reliable transportation.

- Excellent clinical, organizational, and communication skills.

- Ability to work independently and manage time effectively.

-ICU AND CRITICAL CARE EXPERIENCE PREFERRED

-TRACHE AND VENT EXPERIENCE A MUST

-MUST BE ACLS CERTIFIED

Physical Requirements:

- Ability to travel to patient homes.

- Ability to lift and move patients as needed.

- Ability to perform tasks requiring physical activity, including standing, walking, bending, and reaching.
