Full Job Description

Job Summary
We are seeking a dedicated and compassionate Direct Care Team Lead to oversee daily care operations and support a team of Direct Care Staff. The ideal candidate will combine hands-on care experience with strong leadership skills and a commitment to enhancing the quality of life for individuals with developmental disabilities, seniors, and others requiring specialized support. As the Direct Care Team Lead, you will model best practices in care, coordinate staff, and ensure that services are delivered safely, consistently, and in alignment with individualized care plans.

Responsibilities

Leadership & Team Coordination
Provide day-to-day leadership to Direct Care Staff, including assigning duties, monitoring performance, and supporting a positive, collaborative team culture.

Direct Care & Support

Provide direct support to residents with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and meal preparation.
Engage residents in meaningful activities that promote social interaction, cognitive stimulation, and emotional well-being.
Offer compassionate companionship and emotional support, fostering a respectful, dignified, and inclusive environment.

Clinical Oversight & Documentation

Monitor and document resident vital signs and health status, ensuring accurate and timely records in compliance with HIPAA and agency policies.
Observe and report changes in resident behavior, condition, or needs to appropriate healthcare professionals and management.
Administer medications as prescribed, following all medication administration protocols and observing for side effects or adverse reactions.
Ensure all staff maintain proper documentation and shift notes, reviewing entries for completeness and accuracy.

Safety, Compliance & Collaboration

Maintain a clean, safe, and organized living environment, ensuring staff adherence to infection control and sanitation guidelines.
Support the implementation and ongoing review of individualized care plans in collaboration with nurses, case managers, and other professionals.
Promote and enforce compliance with agency policies, state and federal regulations, and industry best practices.
Participate in team meetings, quality improvement initiatives, and incident reviews, providing input on process improvements and resident care strategies.
Serve as the primary point of contact for care staff during the shift, assisting with questions, problem-solving, and decision-making.
Participate in onboarding, training, and mentoring of new staff members, reinforcing policies, procedures, and person-centered care practices.
Coordinate staffing and coverage for breaks, call-offs, and shift changes to ensure adequate support for all residents.
Lead effective shift handoffs, reviewing and communicating important updates, incidents, and changes in resident status.

Qualifications

Previous experience in direct care roles (e.g., home health aide, caregiver, DSP, CNA, or similar) required.
Prior experience in a lead, shift supervisor, or team lead role in a care setting strongly preferred.
Knowledge of patient observation techniques and the ability to recognize and escalate changes in resident behavior or condition.
Familiarity with behavioral challenging clients
Certification in First Aid and CPR is required; behavioral support, or hospice care is a plus.
Strong communication and interpersonal skills, with the ability to provide clear direction, constructive feedback, and supportive coaching to staff.
Ability to perform heavy lifting and assist with resident mobility and transfers safely.
