Full Job Description

Ohio Living Home Health & Hospice is seeking a dedicated and compassionate Registered Nurse (RN) Case Manager to provide exceptional care to patients throughout Central Ohio. This role is ideal for a nurse who is passionate about delivering patient-centered care, coordinating services across the continuum, and supporting patients and families in the comfort of their homes and healthcare facilities.

As an RN Case Manager, you will be responsible for managing patient care plans, performing comprehensive assessments, collaborating with interdisciplinary teams, and ensuring high-quality outcomes for home health and hospice patients.

Essential Duties and Responsibilities

Provides ongoing assessment and nursing care to patients in their homes and healthcare facilities.
Ensures accurate evaluation of patient needs and develops, implements, and manages a comprehensive plan of care that addresses current and anticipated needs.
For home health patients, identifies when patients qualify for or require additional services and notifies the appropriate team member or clinical leader.
Oversees the implementation of the patient's plan of care and evaluates its effectiveness on an ongoing basis.
Follows current nursing professional standards of practice and agency-approved protocols for home health and hospice care.
Provides symptom management and comfort-focused care to support patient goals and quality of life.
Communicates promptly and effectively with physicians and other healthcare providers regarding changes in patient condition and care needs.
Assesses patient and caregiver learning needs and provides education related to disease processes, medications, treatments, safety, and self-management strategies.
Coordinates care with the interdisciplinary team, including therapists, social workers, aides, physicians, and other healthcare professionals.
Maintains accurate, timely, and compliant clinical documentation.
Participates in interdisciplinary team meetings, case conferences, and quality improvement initiatives.
Demonstrates a commitment to delivering exceptional customer service to patients, families, referral sources, and internal team members.
Serves as a patient advocate while promoting dignity, respect, and patient-centered care.
