Full Job Description

Looking for a work life balance? This position is a Monday - Friday with a flexible start time.

As a member of the Zusman Hospice Interdisciplinary Group, the Hospice aide carries out duties assigned by the nurse in order to increase the level of patient comfort, to maintain good personal hygiene and a safe and comforting environment for the patient by performing the duties as described.

Education & Qualifications:

Ability to read, comprehend and write simple instructions, short correspondence, and memos. Ability to effectively present information in one-on-one and small group situations to patients/families/caregivers, and other employees of the organization
Ability to apply common sense understanding to carry out instructions furnished in written, oral or diagram form. Ability to deal with problems involving several concrete variables in standardized situations
Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals
Must possess the ability to manage patient’s behaviors by redirection and/or activity based interventions. Must also be able to deal tactfully with personnel, family members, visitors, agency staff, facility staff and the general public
Must be certified as state tested nursing assistant by the State of Ohio.
Must possess a STNA certificate
Must be able to speak/write the English language in an understandable manner and be able to follow verbal directives
Must have Valid Driver’s license, own liable transportation and proof of auto insurance.

Duties:

Adheres to written and oral instructions for patient care as prepared by the nurse
Checks temperature of all specimen refrigerators and logs results daily.
Assists the patient with all aspects of patient care, ambulation and positioning as required.
Prepares meals and assists with feeding as required
Performs housekeeping services that are necessary for the health and safety of the patient/family/caregiver
Provides guidance to the family/caregiver regarding personal care of the patient
Attends and participates in weekly IDG meetings, in-services, department and agency meetings and other related activities as time allows
Understands and adheres to Zusman Hospice mission, policies/procedures and advocates Zusman Hospice philosophy
Demonstrates a positive attitude and working relationship with staff members and others
Maintains records of services performed and of apparent condition of patient, using documentation guidelines and standards set forth by the facility
Maintains strict confidentiality of patient/family/caregiver information
Adheres to all policies, procedures and regulations regarding patient care, conduct, safety, fire, security and risk management along with operate all required equipment such as wheelchairs and hoyer lifts
Carries out duties and responsibilities of position in a manner that exemplifies excellent customer service
Consistently represents the Facility to all internal and external customers with integrity and professionalism that includes professional appearance in accordance with Facility policy
Regularly greet patient/family by name in a friendly manner
Use positive body language as a means of communicating with patients (i.e. smile, gentle touch, appropriate tone of voice and at eye level during each interaction)
Deliver and receive feedback to others in a respectful, positive and constructive manner
Ensures that Patients’ rights to fair and equitable treatment, self-determination, individuality, privacy, property, and civil rights, including the right to wage a complaint are well established and maintained at all times
Keep the linen cart stocked at all times
Keep supplies for Zusman House and Home Based program stock at all times
