Intake Coordinator (Home Care)

BrightStar Care of Columbus | Columbus, OH

Job Type: Full-Time
Schedule: Monday–Friday, 8:30 AM–5:00 PM
Location: In-Office
Pay: $20–$22 per hour (based on experience)

About BrightStar Care

BrightStar Care of Columbus is seeking an experienced, organized, and detail-oriented Intake Coordinator to oversee the intake, referral, and authorization process for home care clients. This position plays a critical role in coordinating services, securing authorizations, maintaining compliance, and ensuring a seamless transition of care for new and existing clients.

The ideal candidate has experience in home care, home health, healthcare intake, Medicaid authorizations, and care coordination and thrives in a fast-paced healthcare environment.

Key Responsibilities

Receive, review, and process referrals from hospitals, physicians, healthcare providers, and community partners
Serve as the primary point of contact for hospital case managers and referral sources
Manage the full intake process from referral through authorization and start of care
Obtain, track, and maintain Medicaid and other payor authorizations
Monitor authorization expiration dates and renewal requirements to prevent service interruptions
Verify insurance eligibility, coverage, and required documentation
Coordinate with clinical and scheduling teams to ensure timely service delivery
Maintain accurate client records in accordance with company, state, and payor requirements
Ensure compliance with Medicaid, managed care, and other authorization guidelines
Support audits, documentation reviews, and compliance initiatives

Qualifications

Required

Minimum 1 year of experience in home care, home health, healthcare intake, care coordination, or healthcare administration
Knowledge of 485 (Plan of Care) documentation and terminology
Strong communication, organizational, and problem-solving skills
Ability to manage multiple referrals, authorizations, and deadlines simultaneously
High attention to detail and accuracy
Proficiency with electronic documentation systems and Microsoft Office

Preferred

Experience with Ohio Medicaid programs
Knowledge of DODD (Department of Developmental Disabilities) waiver programs
Familiarity with Area Agencies on Aging (AAA) programs
Experience with VA Community Care or Veterans Affairs programs
Previous experience managing healthcare authorizations and compliance requirements
