Overview

Join our compassionate team as a Home Health Aide & Caregiver, where your dedication and caring nature will make a meaningful difference in the lives of individuals needing support at home. This vital role involves providing personalized assistance to promote independence, comfort, and well-being for patients across various stages of life. Whether working with seniors, individuals with disabilities, or those recovering from illness, you will be a trusted partner in their daily care journey. Duties

Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
Monitor patient health by observing vital signs and reporting changes to healthcare professionals
Administer medications following prescribed schedules and medical instructions while maintaining accurate documentation
Support patients with specialized care needs such as Alzheimer's, dementia, hospice, or end-of-life care
Operate Electronic Health Record (EHR) systems to document care activities and medical records accurately
Use medical terminology appropriately when recording observations or communicating with healthcare teams
Employ assistive devices like Hoyer lifts and perform catheter care safely and effectively
Prepare nutritious meals and assist with light housekeeping tasks such as cleaning and laundry to maintain a safe living environment
Follow home healthcare regulations and HIPAA guidelines to ensure patient privacy and safety
Provide emotional support and companionship to promote mental well-being in a home or community setting

Skills

Proven experience in assisted living, long-term care, or home healthcare environments
Strong knowledge of medical terminology, medication administration, and vital signs monitoring
Experience working with individuals with developmental disabilities, Alzheimer's/dementia, or geriatrics is highly valued
Familiarity with EMR/EHR systems for accurate medical documentation
Ability to perform heavy lifting safely and operate mobility aids like Hoyer lifts or wheelchairs
Excellent caregiving skills combined with patience, empathy, and active listening abilities
Knowledge of home & community care regulations and best practices for hospice and end-of-life care
Certifications such as CPR, Basic Life Support (BLS), First Aid, and HIPAA compliance are preferred but not mandatory; training provided if needed
Capable of managing multiple tasks efficiently while maintaining attention to detail in patient care activities
