What You'll Do
As a CDCA Case Manager, you'll serve as an advocate, resource coordinator, and recovery partner for patients throughout their treatment journey.
Responsibilities include:
- Develop individualized care plans that support each patient's recovery goals
- Coordinate treatment services and community resources
- Assist patients with housing, transportation, employment resources, insurance, FMLA, short-term disability, and other recovery-related needs
- Collaborate with probation officers, attorneys, employers, referral sources, and outside agencies as appropriate
- Conduct patient intake, orientation, and ongoing case management services
- Monitor patient progress and update care plans as needed
- Participate in interdisciplinary treatment team meetings
- Support discharge planning and continuum of care coordination
- Engage family members and support systems throughout treatment
- Maintain timely, accurate documentation within the electronic medical record (EMR)
- Help patients overcome barriers to treatment and long-term recovery
What We're Looking For
Required:
- Current Ohio CDCA certification in good standing
- Passion for helping individuals recover from substance use disorders
- Strong organizational and communication skills
- Ability to build rapport with patients, families, and community partners
- Comfortable using electronic medical records (EMR/EHR)
Preferred:
- Bachelor's degree in Social Work, Psychology, Counseling, Human Services, or a related field
- Experience in addiction treatment, behavioral health, or case management
- Knowledge of recovery principles, AA, and evidence-based treatment approaches
- Experience coordinating care across multiple providers and agencies
- Understanding of community resources and continuum of care

Full-Time

- Sunday through Thursday
- Tuesday through Saturday
Hours:
8:30 AM – 5:00 PM
Holiday rotation required.
